Requirements
  • High school diploma or GED is required.
  • Meet Centers for Medicare and Medicaid Services-approved state or national certification requirements within the required timeline.
  • Meet all applicable state licensure, education, and training requirements.
  • Successfully complete the organization's dialysis training program and CPR certification.
  • Be able to perform the physical demands of direct patient care, including lifting and moving patients, prolonged standing, bending, and lifting dialysis solutions.
  • Pass the Ishihara Color Blindness test as a condition of employment; reasonable accommodations may be considered.
Responsibilities
  • Provide safe and effective home dialysis therapy and patient training under the direct supervision of a licensed nurse.
  • Reinforce home hemodialysis and peritoneal dialysis training as permitted by law.
  • Obtain patient vital signs, weights, treatment records, and required intake information.
  • Report missing or abnormal patient, treatment, medication, and laboratory data to the Home Therapies registered nurse.
  • Assist with home visits, home support, patient registration, connected health, supply ordering, and inventory management.
  • Prepare laboratory supplies and specimens for testing, shipping, and transplant-related tracking.
  • Disinfect dialysis equipment and prepare, stock, and maintain examination, training, treatment, and storage rooms.
  • Schedule patient appointments and contact patients or caregivers with dates, times, and reminders.
  • Maintain logs, patient charts, medical records, infection-control records, and audit documentation.
  • Perform clerical duties including answering phones, faxing, email communication, and contacting provider offices.
  • Set up, tear down, program, test, troubleshoot, and disinfect home hemodialysis machines, tools, and connectivity systems.
  • Prepare dialysate and dialysis supplies according to standing or provider orders.
  • Evaluate dialysis access, initiate home hemodialysis treatment, monitor and document patient and machine status, and perform required safety checks.
  • Report significant changes in patient status and document interventions and treatment findings in the medical record.
  • Obtain pre-treatment and post-treatment vital signs and ensure hemostasis of the dialysis access.
  • Assist with water and dialysate testing.
  • Troubleshoot peritoneal dialysis cyclers, modems, and connectivity issues.
  • Perform phlebotomy when directed and when state or competency requirements are met.
  • Set up peritoneal dialysis cyclers, prepare and warm peritoneal dialysis solutions, assist with exit-site care, and track peritoneal dialysis equipment.
