Requirements
  • The candidate must hold a current Physical Therapist Assistant license in the state of employment
  • A minimum of one year of Physical Therapist Assistant experience in an acute care setting is required
  • A valid driver’s license and auto insurance must be held and be listed as a driver
  • Strong communication skills with patients, families, physicians, and colleagues
  • Knowledge of therapy best practices and clinical judgment
  • Ability to assess situations and implement effective solutions
  • Detail-oriented with strong observational skills
  • Strong work ethic with the ability to prioritize and manage time effectively
Responsibilities
  • Provide skilled therapy services in patients’ homes based on the Physical Therapist’s plan of care
  • Observe, document, and report patient responses, progress, and changes
  • Perform prescribed physical therapy procedures and treatments
  • Apply therapeutic techniques to relieve pain, restore function, and improve mobility
  • Assist in assessing functional limitations and implementing interventions to improve patient outcomes
  • Communicate effectively with physicians, therapists, and interdisciplinary team members
  • Participate in case conferences to ensure coordinated patient care
  • Recommend, evaluate, and assist with prosthetic/orthotic devices as appropriate
  • Educate patients, families, and staff on therapy techniques, safety practices, and equipment use
  • Instruct patients and caregivers on the proper use of assistive devices
  • Encourage patient independence and reduce reliance on services when appropriate
  • Complete all clinical documentation in accordance with agency protocols and regulatory guidelines
  • Perform chart audits as requested
  • Communicate scheduling changes promptly
  • Participate in on-call rotations, trainings, meetings, and onboarding of new staff
