Requirements
  • Graduation from an approved certification, program, accredited school, undergraduate school, graduate school, or post-graduate education sufficient to perform the job as described.
  • Hold a valid, unencumbered verification or license to practice the job as described in the state where services are provided.
  • Have the professional experience required by state regulations to perform the job as described.
  • Successfully complete assigned screening and competency testing with a proficiency score of 80% or greater.
  • Have reliable transportation.
  • Provide current tuberculosis results or a chest X-ray.
  • Provide current cardiopulmonary resuscitation certification according to agency policy.
  • Be flexible and organized and exercise independent, sound judgment to perform the job duties and responsibilities.
  • Meet the stated physical requirements, including the ability to sit, stand, stop, bend, kneel, walk, balance, crouch, crawl, push, pull, reach, and perform other physical activities necessary for the job duties.
  • Have vision and hearing capabilities within normal limits, with or without aid.
  • Frequently lift or move up to 15 pounds and occasionally lift or move 100 pounds or more when applicable.
  • Perform the anticipated level of physical assistance required to provide safe client care and notify the supervisor immediately when the required assistance exceeds what can safely be provided.
Responsibilities
  • Provide skilled nursing as ordered on the plan of treatment and nursing care plan.
  • Demonstrate and document clinical skills during each shift or visit in accordance with physician’s orders.
  • Identify and follow through on new problems, changes, and resolutions, including notifying the director or supervisor, obtaining necessary orders for interventions, and documenting them in clinical notes, supplemental physician orders, and the nursing care plan.
  • Observe aseptic technique as required.
  • Perform specific treatments and medication administration according to the care plan.
  • Collaborate as a team member with the patient, physician, and health care team.
  • Identify issues of concern and apply critical thinking skills.
  • Provide skilled nursing care that complies with state and federal regulations.
  • Perform related work as assigned.
  • Participate in mandated agency activities, including in-services, education, and committee work related to patient care.
  • Support and execute the company’s mission, ethics, and goals.
  • Complete all clinical documentation accurately and submit it on time according to agency policies and guidelines.
  • Represent the agency positively and professionally in the company and community.
  • Adhere to the dress code and maintain a clean, neat, professional appearance.
  • Report on time and as scheduled to complete work within designated time.
  • Follow company policies and procedures outlined in the Employee Handbook, Employee Agreement, or communications from the executive team.
  • Travel frequently to complete home health visits in private homes.
