Requirements
  • The candidate must hold a current Registered Nurse license.
  • The candidate must be a graduate from an approved school of nursing.
  • The candidate must have a minimum of two years of experience in an acute setting requiring heart, lung, and physical assessments, or demonstrated ability or education in those areas.
  • The candidate must have current CPR certification.
  • The candidate must have communication, organizational, and human relations skills.
  • The candidate must be able to function with minimal supervision.
  • The candidate must be able to manage multiple projects and changing priorities.
  • The candidate must be able to employ critical thinking skills.
Responsibilities
  • Complete initial and periodic comprehensive assessments of clients, caregivers, and home environments for admission and continued service to determine needs, care effectiveness, and achievement of anticipated outcomes, including applicable OASIS items at appropriate time points.
  • Accept assigned client admissions and weekend on-call assignments.
  • Provide comprehensive nursing care using home assessments, nursing diagnoses, and physician-approved care plans.
  • Perform treatments requiring specialized skills as ordered by the physician.
  • Observe signs and symptoms and report them to the VP of Clinical Services, Director of Professional Services, and/or physician as appropriate.
  • Note changes in clients' physical or emotional status.
  • Assess clients' nutritional needs and diets as ordered by the physician.
  • Educate patients and families on medications and procedures.

HomeRecovery HomeAid provides home health and hospice services for patients who need clinical support where they live. Nurses, therapists, aides and other care professionals assist with recovery, symptom management, daily needs and end-of-life comfort according to each program.
