Requirements
  • Master's degree in Social Work from an accredited graduate school of social work
  • Two years of experience in a medical setting
  • One year of experience within a specialty setting

  • Must hold these licenses/certifications.
Responsibilities
  • Psychosocial Assessment to be completed in a timely manner using theoretical knowledge of human behavior and psychosocial factors affecting treatment outcomes, and understanding ethical/risk issues
  • Clinical intervention including supportive counseling with patients and families, linkage and referral to community agencies and services, facilitating family conferences, helping families cope with difficult decisions (e.g., hospice, DNR, placement), assisting families with communicating concerns to patient’s physicians, and collaborating with Case Managers on discharge planning; provide group facilitation and program development.
  • Triage/Management of clinical caseload demonstrating ability to identify high-risk situations, effectively manage caseload, balance referrals, high-risk admissions, and ongoing patient care; request assistance as needed
  • Incorporation of legal and ethical standards into all clinical assessments and interventions (e.g., suspected abuse or neglect, Tarasoff duty to warn, conservatorship, patient rights); adherence to Tenet Policies, Procedures and Practices in Social Services
  • Documentation completed per policy (Interdisciplinary Plan of Care, Education Form, Advance Health Care Directive follow up form and Social Services Progress Notes)
  • In-service education for other hospital staff regarding psychosocial issues of patient care (e.g., Lunch and Learns)
  • Social Service reports provided in a timely manner (e.g., PAS numbers, case assignment by Social Worker, Absence Approval requests)
  • Quality Improvement participation in development, monitoring and analysis of process and outcome indicators for improvement of patient care
  • Maintains competencies for hospital, age-specific and job-specific standards of care
  • Other duties as assigned (this can include Social Work Month activities, cancer-specific support groups and educational programs for patients, families, staff and volunteers)
