Requirements
  • Education: High School Diploma. CNA preferred. NADSP Certification must be obtained within the probationary period of employment and maintained annually thereafter. AMAP, SCIP, CPR, and First Aid Certifications preferred.
  • Valid NY, PA, CT and NJ Driver’s License with 6 or less points
  • Communication Skills: Effective and practical verbal, written, and technological communication to interact with persons-supported, families/advocates, and other professionals.
  • Problem-Solving Skills: Ability to assess situations, identify potential problems, and develop appropriate solutions.
  • Knowledge of Developmental Disabilities: Comprehensive understanding of the needs, challenges, and best practices in supporting persons with developmental disabilities. Experience working with adults that have developmental disabilities.
  • Regulatory Compliance: Familiarity with regulatory requirements, custodians and standards governing residential programs for persons with developmental disabilities.
  • Organizational Skills: Capacity to organize time effectively, prioritize assignments, and maintain authentic/ contemporaneous records.
  • Professionalism: The ability to handle confidentiality within HIPAA effectively and tactfully with family/advocates, visitors, government agencies, and the public.
  • Teamwork: Ability to work effectively as part of a team, collaborating with coworkers to deliver continuity of care and dignity to those served.
  • Demonstrated strong commitment to safety.
Responsibilities
  • Assists with activities of daily living (ADLs) (e.g., personal hygiene, food preparation, housekeeping, laundry, shopping), as well as mobility, which can be particularly important for individuals with physical limitations or medical conditions.
  • Executes behavioral support (law enforcement, high needs) and monitoring plans, and manages various medical needs (wound, feeding, respiratory care, colostomy, g-tube, frequent medical visits, barrier-free, wheelchair, dementia/Alzheimer's's daily transfers, aging 65+), all while fostering independence and community inclusion.
  • Maintain accurate and timely service records of daily activities, medication administration, behavior data, and any incidents, ensuring compliance with regulations and facilitating communication with other team members. Maintains the facility in compliance with OPWDD, Justice Center, and COA standards.
  • Ensures community inclusion and integration by facilitating participation in community actions, social events, the advisory board, and recreational opportunities, promoting natural supports, CQL standards, and a sense of belonging.
  • Utilizes authorized plans to de-escalate challenging behaviors, Relias, and respond to crises, ensuring the safety and well-being of the individual and others. Participate in Life Plan Meetings and Review Habilitation plans and all oversight documents to ensure value-based outcomes.
  • Provides an atmosphere conducive to positive behavior, while observing, reporting regressive behavior, and working cooperatively with clinical staff.
  • Manages complex aspects of those with dual diagnosis, attending appropriate workshops, training, and meetings.
  • Transport persons to institutions, hospitals, recreational activities, and home visits, and facilitate the day-to-day implementation of needs.
