Join our team and make a positive impact on the lives of others! We are looking for an Intake Specialist I for our team in Pennsylvania.
This is a hybrid role that will require a semi-regular commute to our office in Philadelphia, PA.
PLEASE NOTE:
- Candidates looking to apply must either currently have a HHA or PCA certification, or be willing to gain certification
Department & Position Overview:
The Intake Specialist I will working closely with clients, caregivers and departments across the business to ensure a seamless enrollment process for our patients. Your goal will be to provide a exceptional Customer Service experience, while ensuring that our clients are receiving the care they need. Every Day You Will:
- Ensure a seamless, complete and effective enrollment process for all prospective clients into the PAS program
- Be the initial point-of-contact for calls from prospective clients, caregivers or pending enrollees
- Provide exceptional customer service and positive experience for potential patients/caregivers
- Manage an open caseload until the enrollment process is complete
- Regular communication with a variety of health plans regarding patient benefits and authorizations
- Navigate multiple systems and portals to determine eligibility and obtain authorization
- Regularly achieve monthly new enrollee goals
- Maintain data input, scheduling, document uploading and initiate patient enrollment confirmation or rejection communications
- Be ready and willing to step into the role of an HHA with clients/patients on an emergency basis, if and when needed
Ideal Candidate Will Possess:
- Proven exceptional customer service, case management
- Reliable transportation with the ability to travel to patient homes throughout Pittsburgh and surrounding areas on a weekly basis
- Motivated to help patients/caregivers with the drive to exceed open case targets and metrics
- Must have poise and patience when dealing with clients
- Excellent written and verbal skills
- Computer proficiency required; strong typing skills and ability to leverage technology to identify, research and solve customer issues
- Excellent attention to detail
- Keen attention detail, metrics driven, excellent time management skills
- High empathy and collaborative team player
- Flexibility and openness, as the role and responsibilities may change and/or evolve as we grow!
- Home Health Aide certification required
Nice-to-Haves:
- 2+ years of relevant experience in a similar position is preferred
- Salesforce experience
- 2+ years of customer service experience
- Bilingual in Spanish, or other languages desired

At FreedomCare, base pay is one part of our total compensation package and is determined within a range. This provides our employees with the opportunity to professionally grow and develop within a role. The base pay range for this role is between $16.00 and $20.00 an hour at the commencement of your first year of employment. Compensation decisions are dependent upon a variety of factors which may include, but are not limited to: skill set, years of relevant experience, education, location, and licensure/certifications.
