Requirements
  • Must hold an active Registered Nurse license and/or be a Licensed Social Worker (LSW/LISW) in the state of Ohio.
  • Must have a minimum of two years of recent clinical experience providing direct care to elders or adults with disabilities.
  • Must be able and willing to travel, usually by car, within the assigned geographic area; a valid driver’s license and car insurance are required.
  • Must have experience in case management, care planning, and assessment for eldercare, people with disabilities, and other complex medical needs in a health care setting.
  • Must be able to work in a fast-paced environment while meeting department goals and key performance indicators.
  • Must be able to influence others and work independently with a results-driven approach.
  • Must be proficient in common software and web-based applications, including case management systems and Microsoft Office, and be able to teach others.
  • Additional state-specific qualifications may be required.
Responsibilities
  • Manage the enrollment process for prospective families and educate them about qualifications related to the consumer, caregiver, and home; assess consumer and caregiver eligibility for services.
  • Help consumers apply for Medicaid, waiver programs, and other state benefits.
  • Conduct initial in-person clinical assessments and coordinate service activation while ensuring that all required forms and standard procedures are followed.
  • Assist with caregiver orientation training, including onboarding to Careforth’s HIPAA-secure technology platform and instruction on submitting electronic daily communications.
  • Collaborate to develop each member’s plan of care and review it on an ongoing basis.
  • Provide ongoing coaching to promote effective use of established best practices, caregiver education, and connections to community resources.
  • Review caregiver notes daily and address changes in the consumer’s condition or environment as appropriate.
  • Complete documentation for each on-site visit or encounter.
  • Monitor members’ health status to ensure that their needs and programmatic requirements are properly delivered, including incident management.
  • Collaborate and communicate with the clinical team, agencies, support services, primary care physicians, and other medical providers to ensure timely and appropriate delivery of consumer services.
  • Ensure ongoing compliance with regulatory and accreditation standards, company policies, and procedures.
  • Plan and implement discharges and transitions from the program.
  • Perform other duties as assigned.
