Requirements
  • The candidate must have completed the Modular Education Program for Activity Professionals (MEPAP), hold a bachelor's degree in recreation therapy, be an occupational therapist, COTA, CRT, AT-BC, MT-BC, or possess another clinical degree.
  • Certification in dementia and/or behavior management, such as CDT, is required.
  • The candidate must obtain certification as a Dementia Trainer in Crisis Prevention Management within the first year of hire.
  • The candidate must demonstrate computer skills, including Microsoft Office Word and Excel and web-based applications.
  • The candidate must be able to read, write, speak, and understand English at an intermediate or more advanced level to communicate with residents, coworkers, visitors, and supervisors.
Responsibilities
  • Develop and direct an activity program for residents of a Special Needs Assisted Living Residence, providing entertainment, relaxation, socialization, exercise, stimulation, skill development, and support for residents' individual psychological, cultural, physical, social, and spiritual needs.
  • Train, supervise, and monitor the work progress of Activity Leaders, the Memory Care Program Specialist, and HHAs and their assignments.
  • Plan, direct, and schedule monthly activity programs comprising group, individual, and independent activities tailored to residents' interests and capabilities.
  • Interview new residents, prepare initial resident assessment information, and ensure resident service plans are appropriately updated.
  • Review or attend resident and family care plan review sessions and review resident medical records on the required quarterly, monthly, and annual schedules.
  • Ensure required documentation is completed in residents' files, including activities, activities of daily living, behavior plans of care, initial assessments, progress notes, and activity attendance records.
  • Manage and develop activities within the Seasons model.
  • Provide dementia certification instruction and programming training to staff.
  • Supervise Activity Leaders and the Memory Care Program Specialist and ensure staff follow facility policies and procedures.
  • Direct and monitor resident programming and specialized memory care approaches for personal care duties performed by HHAs, and provide feedback to the Director of Nursing.
  • Plan volunteer programs and recruit, orient, train, and direct volunteers while maintaining relationships with community organizations.
  • Coordinate religious programs and facility-wide entertainment events.
  • Edit the newsletter and collaborate with the Director of Memory Care and Programming and Communications Coordinator on its production.
  • Support volunteers who lead weekly services and facility-wide events and provide entertainment for those events.
  • Communicate with dietary staff regarding residents' diet requirements and supplies needed for activity events.
  • Maintain records and submit money to the Business Office for income-producing activities and resident account transactions.
  • Review and update department policies and procedures annually.
  • Follow facility attendance, dress-code, electronic-device, business-conduct, compliance, and HIPAA policies.
  • Perform other duties assigned by the supervisor, management staff, or Administrator.
Desired Qualifications
  • Qualification as a Therapeutic Recreation Specialist or Activities Professional is preferred.
  • Experience with state and federal requirements and documentation is preferred.
  • Demonstrated supervisory experience is preferred.

Elderwood partners with Trocaire College to advance nursing careers of current employees. Scholarship program provides both financial support and mentorship. Trocaire College in partnership with Elderwood, announced the launch of the Elderwood-Trocaire iCARE Career Pathway Scholarship. This initiative represents a $150,000 annual investment by Elderwood to support up to ten employees at a time as they pursue a practical nursing (LPN), registered nursing (RN), or bachelor's degree in nursing (BSN) at Trocaire. "This new scholarship program is another opportunity for Trocaire and Elderwood to work together to help strengthen the local healthcare workforce," said Dr. Bassam Deeb, president, Trocaire College. "The demand for qualified nurses locally continues, and with Elderwood's commitment to investing in its employees, this program provides financial support, mentorship, and a clear path forward for those looking to advance in their healthcare careers." Beyond financial support, the partnership between Trocaire and Elderwood provides mentorship and guidance from experienced clinical and operational leaders, as well as helping the participants successfully navigate their education and transition into advanced nursing roles. Full-time and part-time academic program opportunities are available. Employees must continue working at least 15 hours per week while enrolled at Trocaire and commit to one-year of employment with Elderwood upon program completion.
