Complex conditions like COPD and congestive heart failure (CHF) are incredibly challenging to manage. People who are already struggling with isolation and the burden of these conditions become increasingly frustrated and disengaged, eventually falling through the cracks and landing in the ER. This cycle perpetuates poor health outcomes and excessive medical costs.

PART TIME 1099 REMOTE POSITION! Up 10-20 hours/week
Where you’ll see your impact:
As a REMOTE Cardiac Rehab Nurse you will specialize in working with our patients who are recovering from various heart diseases and conditions. You will play a crucial role in the rehabilitation process, providing care that helps patients improve their cardiovascular health and prevent future heart issues.
What you’ll be responsible for:
Provide education on heart disease management, including diet, exercise, medication adherence, and lifestyle education.
Support to patients and their families to cope with the stress and lifestyle changes associated with heart disease.
Working with patients remotely
Develop and implement individualized care plans in collaboration with the cardiac rehab team.
AVIALABLE TO WORK 10-20 hours / week M-F during the day time. (days are flexible but will need to be set)
You would be a good fit:
Current Registered Nurse (RN) license
Bachelor of Science in Nursing (BSN) preferred.
Previous experience in cardiac care, rehabilitation, or a related field is highly desirable.
Excellent communication and interpersonal skills.
Compassionate and patient-centered approach to care.
