Requirements
- Current pharmacist licensure through the applicable state board of pharmacy is required: West Virginia, Pennsylvania, Maryland, or Ohio.
- Obtain Basic Life Support certification within 60 days of the hire date.
- Walk and stand for long periods of time.
- Lift 30 pounds and push 50 pounds.
- Use dexterity to finger, handle, and reach.
- Demonstrate time management skills.
- Demonstrate verbal and non-verbal communication skills.
- Demonstrate teamwork skills.
Responsibilities
- Monitor drug therapy by participating in medical rounds or routinely evaluating patients’ medication regimens, medical problems, laboratory data, and special procedures.
- Provide leadership and care for patients in assigned areas, guide providers and patients on best practices and established care guidelines, and assure safe and effective use of drugs through clinical involvement.
- Assist nursing and other disciplines or departments with admission and discharge planning and patient education related to appropriate medication use.
- Provide drug information and educational programs to medical and nursing staff, other healthcare practitioners, and pharmacy staff.
- Maintain professional competence by attending and presenting at conferences, completing continuing education, reading current professional literature, and publishing clinical or practice-related experience or research.
- Participate in administrative, quality assurance, performance improvement, and clinical research activities within the department.
- Participate in medication use evaluations.
- Review each order to verify and confirm accuracy and appropriateness.
- Evaluate compliance with established medication safety procedures.
- Suggest improvements to medication safety based on best practices, including Institute for Safe Medication Practices guidance.
- Document medication errors, unsafe conditions, and near misses.
- Promote an environment focused on medication safety.
- Dispense medications from provider orders.
- Ensure that policies and procedures related to drug distribution are followed.
- Manage drug use control in assigned service areas.
- Identify problems related to drug use control and offer constructive ideas for resolution.
- Make decisions related to the provision of pharmaceutical services in the assigned service area.
- Detect, report, and help minimize or manage adverse drug reactions and medication errors.
- Participate in the management of acute and chronic disease states, medical emergencies, and adverse drug reactions.
- Communicate relevant findings and recommendations and document them in the clinical information system.
- Maintain required records according to legal requirements and hospital policies and procedures.
- Supervise technicians in drug preparation and distribution duties.
- Provide objective information on co-worker performance to facilitate performance management.
- Participate in the orientation and education of pharmacy students, pharmacy residents, and new department employees.
- Participate in educational and scholarly activities related to drug therapy to enhance personal knowledge and that of other health professionals.
- Participate in planning and implementing pharmacy changes and improve productivity through staff meetings and projects.
- Complete and submit written assignments, continuing education documentation, and other assigned items in a timely manner.
- Successfully complete annual clinical competency evaluations.
- Complete annual education related to specialty accreditation requirements where applicable.
Desired Qualifications
- A Doctor of Pharmacy (Pharm.D.) from an Accreditation Council for Pharmacy Education accredited school of pharmacy is preferred.
- Completion of a postgraduate year two residency in an applicable clinical field is preferred.
- Completion of a postgraduate year one residency and board certification in an applicable clinical field is preferred.
- Completion of a postgraduate year one residency and three years of relevant pharmacist experience is preferred.
- Three years of relevant pharmacist experience and board certification in an applicable clinical field is preferred.
