Requirements
  • A high school diploma with at least two years of relevant experience, or a bachelor's degree in a behavioral health or related field.
  • A valid driver's license and reliable transportation for community-based work.
  • Successful completion of a comprehensive background investigation is required for employment.
Responsibilities
  • Deliver community-based support services in homes and other real-world settings, meeting clients where they are.
  • Maintain regular contact with assigned clients through in-person and phone-based support to monitor needs and progress.
  • Review client history, assessments, and treatment plans to ensure services align with individualized goals.
  • Support clients and families in developing and achieving goals outlined in their Individualized Recovery Plan.
  • Teach coping strategies, emotional regulation, and daily living skills to promote independence and symptom management.
  • Help clients identify triggers, build resilience, and develop appropriate behavioral responses.
  • Assist with school, work, and family functioning by reinforcing practical, real-life skills.
  • Connect clients and families to community resources that support basic needs, stability, and long-term success.
  • Support crisis prevention by helping clients develop safety plans and identify support systems.
  • Collaborate with therapists, clinical teams, and community partners to ensure coordinated, whole-person care.
  • Track service utilization and maintain accurate, timely documentation in the Electronic Medical Record system.
  • Participate in team meetings, case staffing, and ongoing training to ensure high-quality service delivery.
