Requirements
  • Candidates must have 5-7+ years of backend engineering experience.
  • Have built and operated production systems that handle real complexity, including distributed stateful workflows and deep integrations with external partners.
  • Comfortable with Kubernetes, Terraform, AWS, and CI/CD, and capable of deploying code and owning system reliability.
  • Experience using frontier agentic tools to speed up development and ability to audit and verify outputs from AI models.
  • Strong collaboration and communication skills, with ability to work in cross-functional teams where healthcare regulations, compliance, and patient safety shape engineering requirements.
  • Willingness to work in-person at the San Francisco office, five days a week.
  • For Staff leveling: demonstrated track record of setting technical direction across teams and lifting engineers around you.
Responsibilities
  • Architect Healthcare Commerce by building and evolving a subscription and billing infrastructure that handles the realities of healthcare, including mid-cycle medication switches, dynamic pricing across pharmacy partners, payment recovery, and prorated billing.
  • Build Clinical State Machines to power complex medication ordering and fulfillment, including compounding pharmacy integrations, controlled substance compliance, and personalized dosing protocols across a distributed network of external partners.
  • Own the Infrastructure by taking hands-on ownership of AWS and Kubernetes infrastructure, establishing CI/CD pipelines, building observability, and creating deployment patterns for rapid shipping.
  • Act as a Product Partner by working directly with Clinical, Operations, and Product teams to shape workflows, owning business outcomes, and delivering maximum patient value with efficient implementation.
  • Build Agentic Workflows by designing backend architecture that enables AI-assisted agents to automate clinical operations, improve patient safety, and scale provider workflows.
  • Raise the Engineering Bar by making key architectural decisions, upgrading legacy systems, and elevating engineers through rigorous code reviews, testing practices, and technical standards.
Desired Qualifications
  • Experience with the stack including TypeScript, Node.js, Express, PostgreSQL, Redis, Kubernetes, Terraform and AWS is a strong plus, with emphasis on fundamentals, distributed systems experience, and domain reasoning over language syntax.
  • Experience designing and deploying healthcare subscription and billing infrastructures, including mid-cycle medication switches, dynamic pricing across pharmacy partners, payment recovery, and prorated billing.
  • Experience building clinical state machines for medication ordering and fulfillment, including compounding pharmacy integrations, controlled substance compliance, and personalized dosing protocols.
  • Experience building AI-assisted backend architectures and agent-based workflows to automate clinical operations and scale provider workflows.
  • Knowledge of healthcare regulations, compliance, and patient safety considerations.
