Requirements
  • Good working knowledge of Medicare Part A and certification procedures.
  • A Bachelor of Social Work or LVN qualification as a licensed professional.
  • Effective oral, written, and interpersonal communication skills.

Responsibilities
  • Obtain referrals from hospital social workers, physicians, or family members and keep wait lists current.
  • Conduct admission interviews with residents or responsible parties and obtain required financial, signature, social, medical, attending physician, and other admission documentation.
  • Review admission paperwork with residents or responsible parties, including Medicare Part A coverage and non-coverage procedures.
  • Maintain nonmedical-record admission paperwork in an admissions file for each resident.
  • Maintain ongoing daily and monthly census activity, including admissions, discharges, deaths, room transfers, payment-method changes, and temporary or permanent assignments.
  • Maintain an accurate record of the number of days an independent living resident resides in the Health Center.
  • Coordinate utilization review for new admissions covered by Medicare Part A.
  • Assist in coordinating admissions, transfers, and discharges with other departments.
  • Ensure that residents, families, and responsible persons are fully informed of residents’ personal and property rights.
  • Communicate professionally with internal and external contacts.
  • Use computers and computer programs, a copy machine, fax machine, calculator, and telephone.
  • Abide by Carillon policies and procedures, the Code of Ethics, and HIPAA requirements.
Desired Qualifications
  • Experience in a long-term care setting.
  • One year of Admissions Coordinator experience in a healthcare setting.
