- A Master's degree in social work from a school of social work accredited by the Council on Social Work Education is required.
- An active Ohio Social Work license is required.
- A valid driver's license and reliable transportation insured according to Ohio requirements are required.
- At least one year of experience in a health care field is required, including experience working with patients and families coping with terminal illness and dying.
- The role requires effective communication, comprehension, documentation, interpersonal, and computer skills.
- The candidate must be able to assess patient needs and formulate individualized patient care plans.
- The candidate must be able to work in a field setting and make sound judgments.
- Conduct an initial comprehensive psychosocial and bereavement risk assessment of the patient, family, and caregiver within five calendar days after the patient's election of hospice care.
- Obtain and make referrals to community resources, including financial assistance, and provide appropriate follow-up.
- Assist families and caregivers with placement of patients in basic care, respite, or inpatient facilities when appropriate.
- Provide psychosocial and anticipatory grief counseling to patients, families, caregivers, and significant others regarding the impact of terminal disease and patient care.
- Assess the psychosocial status of patients and families in relation to coping capacity, terminal illness, environment, and death; provide counseling, assist with community resources, and communicate findings to the team.
- Participate in developing plans of care and treatment and hold case conferences as needed.
- Prepare social histories and evaluations and develop intervention plans based on findings.
- Update, maintain, and submit accurate and timely clinical documentation according to agency guidelines and assist with discharge planning.
- Attend interdisciplinary team meetings to coordinate care plans, follow up on changes, solve problems, identify social problems and their severity and interrelatedness to medical situations, assess family strengths and weaknesses, and discuss alternative methods of alleviating problems.
- Help team members understand significant social and emotional factors affecting patients and families.
- Provide care respectfully and with dignity, and report complaints, abuse allegations, misappropriation of patient property, and other corporate compliance matters.
- Teach patients, families, and caregivers about disease progression and self-care techniques, including counseling support and instructions ordered by a physician.
- Hospice or home care experience is preferred but not required.
- Act as a mentor to other personnel by participating in orientation and assisting with field training and in-service education for contractors and referral sources.
