Requirements
  • Graduate of an approved nursing program as an LPN or RN.
  • Hold a current Ohio Licensed Practical Nurse or Registered Nurse license.
  • Demonstrate strong organizational skills and system development.

Responsibilities
  • Oversee daily Assisted Living operations, including supervision, scheduling, assignments, staffing, productivity, accountability, orientation, and training of staff.
  • Complete Assisted Living 90-day and annual evaluations.
  • Plan and maintain consistent, sufficient staffing to meet resident needs.
  • Participate in recruitment and retention of staff.
  • Support staff in assessment, care planning, intervention, and evaluation of residents.
  • Provide wellness services and hands-on floor assistance during staffing challenges.
  • Ensure residents' medical, psychosocial, emotional, and spiritual needs are addressed and documented in the Plan of Care.
  • Ensure the area, equipment, and supplies are clean, safe, and used appropriately.
  • Ensure campus and department policies, procedures, guidelines, and local and state regulations are followed.
  • Participate in interdisciplinary team activities and provide timely follow-up and feedback.
  • Coach and teach staff to communicate their needs and residents' needs.
  • Ensure exceptional customer service.
  • Ensure resident physicals, drug screens, and tuberculosis screens are provided.
  • Schedule cardiopulmonary resuscitation and first-aid training and track attendance for compliance.
  • Conduct new-resident assessments and attend resident care conference meetings.
  • Update incident reports with interventions, document progress notes, enact interventions, and sign incident reports.
  • Coordinate transitions from independent living to assisted living and long-term care.
  • Attend scheduled meetings and daily morning health-services leadership meetings.
  • Report details and analysis of unplanned hospitalizations, emergency-room visits, and falls.
  • Review progress notes and report relevant information to the team.
  • Conduct daily staff huddles to relay pertinent real-time information.
  • Meet weekly with the Director of Health Services to establish common goals and evaluate progress.
  • Complete weekly shepherd visits.
  • Serve as the on-call nurse in rotation with the Director of Nursing.
  • Conduct periodic staff meetings and evening, night, and weekend visits.
  • Develop assisted-living and wellness processes and procedures as needs arise.
  • Communicate and build positive relationships with residents and family members.
  • Assist with referral management.
Desired Qualifications
  • Previous experience in a continuing-care community.
  • Two years of healthcare experience.
