Requirements
  • An active Licensed Practical Nurse license for the applicable state is required.
  • Three years of experience in a related organization is preferred.
  • The ability to observe and communicate effectively, both in writing and verbally, is required.
  • The ability to work independently with little supervision is required.
  • The ability to be flexible and cooperative in fulfilling obligations is required.
  • The ability to solve problems, make ethical recommendations, and define appropriate boundaries is required.
  • Compliance with accepted professional standards and practice is required.
  • Comfort using computers and basic skills with computer platforms such as Word and Excel are required.
Responsibilities
  • Assist residents in achieving their health and wellness goals within the framework of the eight dimensions of wellness, personal choice, and mind-body-spirit.
  • Provide support strategies and referrals for chronic disease management and acute illness or injury, and serve as a resource for supportive options that improve health, function, and lifestyle.
  • Develop a referral network of service providers inside and outside the community in collaboration with the Director of Resident Services.
  • Participate in collaborative weekly care meetings and work with the multidisciplinary team to strategize for resident outcomes, campus flow, and occupancy.
  • Oversee post-acute discharge planning and follow-up support, including coordinating admissions, transfers, and discharges with health center and hospital staff.
  • Attend medical emergency calls while on duty and partner with residents on risk-reduction strategies to prevent further incidents.
  • Complete incident reports according to community protocol.
  • Provide annual updates for medical demographic information and advance directives.
  • Organize and facilitate vaccination clinics.
  • Encourage utilization of and support services provided by on-site healthcare practitioners.
  • Partner with the Community Life Director and Resident Services Director to plan, implement, and evaluate wellness programming, health screenings, and health education opportunities.
  • Build relationships with residents and establish trust, respect, and connection.
  • Communicate with families and staff and drive measures to improve community culture.
  • Maintain records appropriate for independent living and protect the confidentiality of resident information.
  • Attend assigned in-service training and educational sessions.
  • Work to achieve high levels of resident satisfaction and apply sound judgment and organizational skills.
  • Perform other duties assigned by the Resident Services Director.
