Requirements
  • The candidate must hold a current license from the State Board of Nursing as a Registered Nurse.
  • The candidate must have at least three years of nursing experience.
  • The candidate must be able to plan, evaluate, and coordinate services.
  • The candidate must have experience triaging tasks, supervising nursing team members, and coordinating services to provide an optimum resident and patient experience.
Responsibilities
  • Receive reports on patient and resident status from the previous shift, evaluate needs, and ensure appropriate resources are in place.
  • Conduct physical rounds to assess quality of care, intervene by providing care, and interact with caregivers and patients or residents.
  • Provide leadership and support to care team members, including timely medication passes, physician- or Registered Nurse-directed care plans, documentation, and treatments.
  • Evaluate the performance of direct-care team members and inform the Director of Nursing Services about coaching or corrective-action opportunities; assist with investigations as needed.
  • Evaluate and monitor whether patient and resident needs are met and whether policies and procedures are followed.
  • Evaluate critically ill residents, report findings to physicians, record observations, provide direct care as needed, and notify the Director of Nursing Services and Administrator according to policy.
  • Respond to and coordinate actions during emergencies and notify the Director of Nursing Services and Administrator according to policy and procedure.
  • Work with the Director of Nursing Services to facilitate solutions to systemic or procedural concerns.
  • Ensure safe work practices are followed.
  • Communicate empathetically with families, patients, and residents about changes in condition and care concerns.
  • Ensure resident rights are respected.
  • Work directly with care team members to ensure care is provided.
  • Perform other duties assigned by the Director of Nursing Services or Administrator.

  • Experience in the long-term care industry is preferred.
