Form Health named to Fast Company's annual list of the World's Most Innovative Companies of 2026.

Form Health named to Fast Company's annual list of the World's Most Innovative Companies of 2026. 3 minutes read Form Health joins the ranks of Google, Nvidia, Adidas, Walmart, and more BOSTON-(BUSINESS WIRE)-Form Health, the national leader in science-based obesity and cardiometabolic care, is proud to have been named to Fast Company's prestigious list of the World's Most Innovative Companies of 2026. This year's list shines a spotlight on businesses that are shaping industry and culture through their innovations. Alongside the World's 50 Most Innovative Companies, Fast Company recognizes 720 honorees across 59 sectors and regions. The full list of healthcare honorees is available here. "Obesity and cardiometabolic conditions represent some of the most urgent and costly health challenges facing employers and the entire healthcare system," said Evan Richardson, founder and CEO of Form Health. "At Form, we've built a model for treating patients that combines physician-led care with responsible, severity-aware prescribing of medications, grounded in clinical rigor and delivering measurable ROI. The result is better outcomes for patients and a more sustainable way for employers to expand access to care, while keeping costs predictable. We're honored to be recognized by Fast Company as one of the world's most innovative companies. This recognition reflects the tireless work of our team to transform how obesity and cardiometabolic care are delivered to patients, every day." Form Health has pioneered an approach to obesity and cardiometabolic care that helps employers navigate the rapid rise of GLP-1 medications, while expanding access responsibly and keeping costs predictable. Rather than treating these breakthrough medications as a standalone solution, Form integrates them into comprehensive physician-led care that combines nutrition, physical activity, behavioral support and medication, where appropriate. Through severity-aware prescribing protocols and structured care pathways, Form ensures that patients with the greatest clinical need can access medications as part of personalized medical care, while also providing access to effective, lower-cost therapies and intensive lifestyle interventions. This approach improves real-world outcomes while giving employers a more predictable and cost-effective way to expand access to evidence-based treatment. Building on this model, Form recently expanded its offering to include a comprehensive diabetes program, creating a single integrated solution for cardiometabolic care. The program unifies obesity and diabetes treatment under a single physician-led cardiometabolic framework for employers, replacing fragmented point solutions with continuous medical care designed to improve outcomes and reduce long-term costs. Form is also working with leading pharmaceutical innovators to create new pathways for responsible access to obesity medications, including joining Eli Lilly's Employer Connect platform and Novo Nordisk's NovoCare(R) platform. Together, these innovations are helping employers move beyond fragmented wellness programs and toward a clinically rigorous model for managing obesity and related chronic diseases that improves health outcomes while delivering measurable ROI. The World's Most Innovative Companies is Fast Company's hallmark franchise and one of its most anticipated editorial efforts of the year. To determine honorees, Fast Company's editors and writers review companies driving progress around the world and across industries, evaluating thousands of submissions through a competitive application process. The result is a globe-spanning guide to innovation today, from early-stage startups to some of the most valuable companies in the world. "Our list of the Most Innovative Companies is about spotlighting organizations that don't just adapt to change - they drive it," said Brendan Vaughan, editor-in-chief of Fast Company. "The companies we honor this year are redefining what leadership looks like in 2026, pairing bold ideas with measurable impact and turning breakthrough innovation into real-world value. They are setting the pace for their industries and offering a blueprint for what sustained innovation can achieve." The full list of Fast Company's Most Innovative Companies honorees can now be found at fastcompany.com. It will also be available on newsstands beginning March 31, 2026. Fast Company will host the Most Innovative Companies Summit and Gala for honorees on May 19 in New York City. The summit features a day of inspiring content, followed by a creative black-tie gala including networking, a seated dinner, and an honoree presentation. Form Health launches diabetes care programme alongside obesity offering for US employers

Form Health, a US-based obesity care provider, has launched a diabetes programme, expanding its offering into an integrated cardiometabolic solution for employers. The new service provides physician-led care for employees with Type 2 diabetes and prediabetes, including medical oversight from American Board of Obesity Medicine-certified doctors, support from registered dietitians and medication management. The programme addresses both obesity and diabetes through continuous medical care, rather than treating them separately. Approximately 90% of people with Type 2 diabetes live with overweight or obesity, making the integrated approach particularly relevant. Built on Form's existing clinical infrastructure, the diabetes offering combines team-based care with connected devices and digital tools for glucose and nutrition monitoring. Employees can enrol with a diabetes or prediabetes diagnosis without meeting specific BMI thresholds.

Form Health Launches Diabetes Care, Expanding Its Science-Based Obesity Program Into a Single, Integrated Cardiometabolic Solution for Employers Nationwide

Form Health launches diabetes care, expanding its science-based Obesity Program into a single, integrated cardiometabolic solution for employers nationwide. Provided by Business Wire Mar 16, 2026, 5:00:00 AM Building on Proven Employer ROI and a Clinically Rigorous, Outcomes-Driven Obesity Program, Form Is Extending Its Industry-Leading Care Model to Employees With Diabetes Today, Form Health, the national leader in science-based obesity care, announced the launch of its new diabetes program, expanding its proven obesity offering into a single, integrated cardiometabolic solution for employers. The diabetes offering delivers personalized, physician-led care for employees with Type 2 diabetes and prediabetes, including American Board of Obesity Medicine (ABOM)-certified medical oversight, dedicated support from a Registered Dietitian, and medication management, as clinically appropriate. The expansion reflects growing demand from employers seeking a more unified approach to cardiometabolic health. Rather than separating weight management from diabetes support, Form's model treats these conditions through continuous, physician-led medical care designed to address root causes, manage risk over time, and deliver measurable improvements across populations. Diabetes alone affects more than 30 million Americans, and approximately 90% of people with type 2 diabetes are living with overweight or obesity. By addressing obesity as a root cause of cardiometabolic disease progression, Form's clinical-first approach improves outcomes at scale while helping employers manage long-term medical costs. "For far too long, employers have been left to piece together costly obesity and diabetes programs without solutions that actually work," said Evan Richardson, founder and CEO of Form Health. "We built our obesity care model to provide that clarity, delivering guaranteed ROI and industry-leading clinical outcomes. Now we're extending that same proven model to diabetes. We are giving employers the chance to uplevel the support they offer their employees, providing responsible, medical care, not just more behavioral hoops to jump through. This solution is a single, integrated way to manage cardiometabolic health, grounded in science and delivering results they can see and trust." Built on the same clinical and technological infrastructure that underpins Form's obesity program, the diabetes offering combines team-based care provided by ABOM-certified physicians, advanced practice providers, registered dietitians, and condition-specific care pathways with integrated data systems that allow clinicians to proactively monitor progress and adjust treatment. The program incorporates connected devices and digital tools, including glucose data, hemoglobin A1C tracking, and nutrition monitoring, giving care teams a comprehensive, real-time view of patient health. The program applies a population health approach to detect, stratify, intervene, and measure outcomes, integrating clinical data and tracking key diabetes metrics. Employees can enroll with a diabetes or prediabetes diagnosis, and for these new pathways, do not need to hit specific Body Mass Index (BMI) thresholds to join. Unlike point solutions or wellness programs, this model ensures measurable health outcomes, closes care gaps, and delivers predictable ROI, bringing the same clinically rigorous, science-backed approach that Form has delivered in obesity care. Obesity and related cardiometabolic conditions represent one of the largest drivers of healthcare costs for employers and health plans. Cardiometabolic conditions such as obesity, type 2 diabetes, and prediabetes respond to the same core fundamentals: nutrition, physical activity, behavioral health support, and medication, when appropriate. Form's established care model integrates all four pillars into a comprehensive treatment plan delivered by dedicated clinical teams, ensuring whole-person care rather than fragmented point solutions. Florencia Halperin, MD, MMSc, Chief Medical Officer at Form, added, "Many solutions in this space focus on coaching or looking backward at data. Our model embeds medical decision-making, prescribing, and continuous oversight directly into the benefit so employers can move beyond participation metrics and toward measurable improvements in health outcomes and total cost of care; and so patients can rely on Form as their partner in long-term cardiometabolic health."
