- An active Michigan occupational therapy license in good standing is required.

- Competency in core home health occupational therapy scope, including activities of daily living and instrumental activities of daily living training, home safety, adaptive equipment, and caregiver education, is required.
- Complete in-home occupational therapy evaluations and deliver skilled occupational therapy visits.
- Build and execute plans of care focused on activities of daily living, instrumental activities of daily living, safety and fall prevention, and functional independence.
- Recommend and provide training on adaptive equipment and home modifications as needed.
- Coordinate with nursing and the broader care team when a patient's condition or needs change.
- Document promptly and thoroughly in a defensible, efficient, and consistent manner.
- Carry a full, high-quality caseload and ramp to approximately 28 productivity points per week at the target visit quality.
- Drive low avoidable-hospitalization rates and strong patient-experience scores, and participate in case conferences and team huddles to improve quality.
- Build trust with referral sources through responsive and high-quality patient care.
- Help establish clinical, documentation, cadence, and judgment standards as the care pod grows.
- Home health experience is strongly preferred.
- Prior home health experience is preferred.
- Deeper experience with neuro, cerebrovascular accident, fall-risk, dementia, or caregiver-training occupational therapy populations is preferred.
- Strong organization and time-management skills are preferred.
