Requirements
  • Graduate of an accredited school of professional nursing.
  • Hold a current license to practice as a Licensed Practical Nurse or Licensed Vocational Nurse in the state(s) of employment.
  • Maintain current cardiopulmonary resuscitation certification.
  • Have a valid driver's license and appropriate automobile insurance coverage under the agency policy.
  • Be available and on time for scheduled work shifts.
  • Undergo a drug test and initial and continuous exclusion and sanction or disciplinary monitoring.
  • Meet any additional eligibility requirements based on the specific position.
  • Be able to exert up to 35 pounds of force and perform sitting, walking, standing, driving, talking, hearing, reaching, desk work, and prolonged phone-related activities.
Responsibilities
  • Perform skilled nursing functions in accordance with applicable federal, state, and local laws, the Nurse Practice Act, professional standards, and agency policies.
  • Provide professional nursing care under the direction of a Registered Nurse and communicate client status or condition changes to the RN.
  • Provide skilled nursing visits as directed by the RN Case Manager and follow the established care plan.
  • Recognize and report changes in client condition to the Case Manager and physician.
  • Communicate information promptly through verbal communication and complete, accurate clinical documentation.
  • Provide personal care services, monitor and report findings, administer and monitor medications, teach clients, perform emergency interventions, and provide nursing services within the professional scope of practice as delegated by the Case Manager.
  • Promote personal safety and a safe environment for clients.
  • Follow safety and infection-control practices and recognize and respond appropriately to potentially unsafe conditions.
  • Make recommendations for improvement of agency practices and seek education, training, or assistance for areas of concern.
  • Accept responsibility for professional nursing practice and follow applicable laws and agency requirements.
  • Maintain confidentiality and refrain from discussing client or staff identities, conditions, or circumstances with unauthorized people.
  • Participate in development, implementation, and evaluation of the Agency Quality Improvement Program and related activities.
  • Participate in coordination of home care services, attend required meetings, and follow reporting requirements.
  • Complete all mandatory and regulatory training programs.
  • Perform other duties as assigned.
