Requirements
  • B.A. or B.S. degree in psychology, counseling, social work, gerontology, or related field with 3 years of relevant experience or an equivalent combination of education and experience
  • Experience working with high needs or high acuity populations that have complex medical needs and/or have experienced trauma, homelessness, or any combination of the aforementioned
  • Experience working with CalAIM Housing Suite of Community Supports, Community Health Worker, and/or Enhanced Care Management
  • Experience using and administering multiple assessment tools and interpreting information/data derived from assessments
  • Experience supporting program and service implementation and coordination
  • Experience collaborating with local service providers
  • Strong written/verbal communication; able to navigate language barriers using translation tools; bilingual needs vary by property
  • Sound judgment, discretion, and effective problem-solving when working with participants and partners
  • Skilled in relationship-building, active listening, negotiation, and conflict resolution while maintaining professional boundaries
  • Detail-oriented with strong organizational skills; able to work independently and collaboratively
  • Knowledge of common mental health conditions and interventions including harm reduction, motivational coaching, and trauma-informed practices
  • Continuous learning mindset and commitment to best practices in serving high-acuity populations
  • Proficient with Outlook, OneDrive, Teams, Zoom, Salesforce, and Microsoft Office tools
  • Valid CA driver’s license, reliable transportation, and ability to travel between properties (at least 60% field-based)

Responsibilities
  • Implement CalAIM HTSS services at assigned properties and uphold all program standards and compliance requirements
  • Provide direct case management to HTSS participants, supporting housing stability, goal setting, and access to needed resources
  • Complete assessments, documentation, and required reporting while maintaining strict adherence to HIPAA policies
  • Respond to crisis situations and carry out mandated reporting procedures as needed
  • Support continuity of care by providing coverage across properties during staff vacancies or time off
  • Collaborate with Property Management, Resident Services, Managed Care Plans, and community partners to address participant needs and improve service coordination
  • Deliver high‑quality customer service through responsive communication, needs-based follow‑up, and partnership with external providers
  • Maintain accurate Salesforce records—including data entry, progress notes, and participant plans—and use data to inform decisions
  • Participate in team meetings, trainings, and collaborative activities while modeling a positive, solution‑focused approach
  • Contribute to special projects, working groups, and continuous improvement efforts as assigned
