Requirements
  • A degree in a behavioral health-related field or comparable experience.

Responsibilities
  • Provide housing coordination services that enable Young Adult Services clients to transition from structured residential placement to independent community living.
  • Provide services in the general area of the local mental health authority and other areas requested by local mental health authority project managers and approved by the contractor.
  • Work with landlords and real estate agents to develop and maintain a list of landlords with available apartments.
  • Participate in securing and negotiating leases entered into by clients.
  • Ensure lease agreements comply with the policies and procedures in the Department of Mental Health and Addiction Services Young Adult Services Client Support Funds Services manual.
  • Provide copies of fully executed leases to the applicable local mental health authority project managers.
  • Maintain lists of residential support sites and scattered-site housing vacancies.
  • Oversee and conduct inspections required by Young Adult Services Client Support Funds Services policies and procedures.
  • Handle damage claims, inspections, and determinations needed to resolve them, including coordinating with the Young Adult Services treatment team to involve clients when necessary.
  • Coordinate and communicate with the Young Adult Services treatment team regarding pending evictions and possible vacancies.
  • Oversee and coordinate infestation remediation by ensuring landlords treat infestations within the required timeframe.
  • Address housing issues for Young Adult Services clients and communicate needs, resources, and case-specific issues to the treatment team.
  • Maintain working knowledge of available housing and utility assistance programs in the local mental health authority area.
  • Assist clients with applications for eligible housing and utility assistance benefits and maintain records of benefit dates, amounts, and related documentation.
  • Work with the Financial Coordinator and Young Adult Services team on budgets for clients receiving rent-related subsidies.
  • Prepare and submit the monthly Young Adult Services Housing Financial Assistance report to the Department of Mental Health and Addiction Services.
  • Represent the organization and its services professionally and follow established behavioral healthcare policies, procedures, and best practices.
