Requirements
  • The candidate must hold a current Registered Nurse license in good standing with the state nursing board.

Responsibilities
  • Conduct initial and ongoing comprehensive assessments of the impact of terminal disease on patients’ physical, functional, psychosocial, and environmental needs using documentation, clinical records, case conferences, team reports, evaluations, and activities of daily living.
  • Apply admission and re-certification criteria for hospice care to establish appropriate levels of care and patient eligibility.
  • Implement, develop, and document plans of care to ensure quality and continuity of care, and recommend revisions as necessary.
  • Communicate effectively with patients, families, team members, and other health care professionals.
  • Apply infection-control concepts and standard precautions while coordinating and performing patient-care activities to prevent contamination and disease transmission.
  • Provide clinical direction to hospice aides and licensed practical or vocational nurses to ensure quality and continuity of services.
  • Prepare clinical and progress notes demonstrating progress toward established goals and ensure continuity of quality patient care through appropriate documentation.
  • Inform physicians and other personnel of changes in patients’ needs and intervention outcomes, and evaluate patient and family responses to care.
  • Monitor assigned cases for compliance with third-party payor requirements.
  • Demonstrate professional growth and competency and promote the agency’s philosophy and administrative policies.
  • Consult with and educate patients and families about disease processes, self-care techniques, end-of-life care, nutrition, and dietary needs.
  • Prioritize patient and family needs with members of the interdisciplinary group and provide training to other staff as needed.
  • Determine the scope and frequency of services based on acuity and patient and family needs, and assess caregivers’ ability to meet patients’ immediate needs upon admission and throughout care.
  • Initiate appropriate preventive and rehabilitative nursing procedures.
Desired Qualifications
  • Knowledge of or experience using the Homecare Homebase electronic medical record system.
  • Experience in home health or a related health care setting.
