Requirements
  • Strong leadership qualities, including communication skills, the ability to provide on-call support in a 24-hour setting, strong ethical standards, and the ability to promote a culture of learning and growth.
  • Maintain current CPR and first aid certification and complete Medication Administration and other training required by the state, accreditation body, or agency.
  • Working knowledge of Microsoft products.
  • Bachelor's degree in rehabilitation, psychology, or a related major; equivalent experience and extraordinary performance may be considered in lieu of the degree.
  • Three years of experience in the human services field, including one year of supervisory experience.
  • Must be at least 21 years old.
  • Valid driver's license, acceptable driving record, agency driver-insurability eligibility, reliable vehicle, and current automobile insurance.
  • Ability to perform significant bending, lifting, and stooping; alternate between sitting and standing as dictated by client needs; lift or transfer individuals and physically assist clients with movement and care; lift at least 35 pounds; and perform repetitive physical activity.
Responsibilities
  • Advance Tangram’s mission, strategic plan, and values.
  • Provide direction, leadership, performance feedback, and training for assigned staff.
  • Build and facilitate positive relationships with clients, natural supports, teams, professionals, and community members.
  • Participate in ongoing professional development and promote the continued development of team members.
  • Identify opportunities to add new clients, increase occupancy, and expand services to support program growth.
  • Oversee Community Living Managers across multiple locations, ensuring consistent monitoring and implementation of client support plans, review of outcomes and documentation, and regular reporting on client progress.
  • Ensure client health and safety through medication-administration review, follow-up on medical appointments, emergency planning, site inspections, and emergency drills.
  • Prepare staff to provide client care and support through direct and coordinated training and collaboration with other professionals.
  • Ensure accurate service allocations and utilization of client resources.
  • Obtain and complete admission, enrollment, and training documents for clients.
  • Participate in and help guide Person-Centered Individualized Support Plan team meetings.
  • Create meeting notes for client meetings.
  • Create monthly reports and upload them to the Bureau of Disabilities portal.
  • Maintain client electronic and in-home records, including contacts, financial data, and medical appointments.
  • Respond to and report critical issues, incidents, and emergent needs.
  • Provide direct client support as needed, including transportation to appointments, community engagement, and personal care.
  • Oversee and implement entitlement systems related to Social Security, Medicaid, Medicare, and Section 8 housing, providing required documentation.
  • Maintain compliance with service standards and accreditation requirements.
  • Participate in recruitment, hiring, onboarding, performance evaluations, and ongoing feedback to maintain team performance and engagement.
  • Review and approve staff schedules to ensure adequate coverage, manage staffing levels to minimize overtime expenses, and use resources efficiently.
