Requirements
  • A minimum of a Master's Degree in Social Work, Counseling, or a related field is required.
  • Two years’ experience working with youth or families.
  • Availability for 24/7 on-call support for assigned families as needed.
  • Completion of required Family Centered Treatment certification within the first year.
  • Successful completion of a comprehensive background investigation is required for employment.
Responsibilities
  • Provide Family Centered Treatment services in alignment with model fidelity requirements and clinical service definitions.
  • Deliver in-home and community-based therapy to children, adolescents, and families using a structured, phase-based treatment model.
  • Develop, implement, and adjust service plans in collaboration with treatment teams, families, and other involved systems.
  • Facilitate family engagement, goal setting, psychoeducation, and skill-building aligned with identified treatment needs.
  • Coordinate care with schools, medical providers, child welfare, juvenile justice, and natural supports to ensure integrated service delivery.
  • Maintain accurate, timely documentation, complete all required Family Centered Treatment forms, and support discharge planning and aftercare coordination.
  • Work directly in clients’ homes and other natural environments, often outside traditional business hours.
  • Carry a caseload requiring regular engagement with assigned families.
  • Participate in ongoing training and fidelity monitoring as part of the Family Centered Treatment model requirements.
Desired Qualifications
  • Current Family Centered Treatment Certification is preferred.
