Requirements
  • A minimum of a Nursing Degree from an accredited college or university, or graduation from an approved Licensed Practical Nurse/Licensed Vocational Nurse program.
  • A current, unencumbered, active license to practice as a Licensed Practical Nurse/Licensed Vocational Nurse in the state.
  • Ability to read, write, speak, and understand English; read technical procedures and policy and procedure manuals; present information; and respond to questions from managers and employees.
  • Ability to apply fractions, percentages, ratios, and proportions to practical situations.
  • Knowledge and skills necessary to provide care appropriate to the age-related needs of residents.
  • Ability to make independent decisions when circumstances warrant action.
  • Ability to deal tactfully with personnel, residents, family members, visitors, government agencies, and the general public.
  • Knowledge of nursing and medical practices and procedures, and of laws, regulations, and guidelines pertaining to nursing care facilities.
  • Leadership and supervisory ability, including willingness to supervise other personnel.
  • Ability to plan, organize, develop, implement, and interpret programs, goals, objectives, policies, and procedures needed to provide quality care.
  • Ability to meet the facility's general health requirements, including a medical and physical examination.
  • Ability to move intermittently, push, pull, move, and/or lift at least 25 pounds to a minimum height of 5 feet and carry that weight at least 50 feet.
  • Current CPR certification.
  • Ability to maintain required continuing education and licensing and remain in good standing with the State Board of Nursing.
Responsibilities
  • Provide direct nursing care to residents and supervise the day-to-day nursing activities performed by nursing assistants.
  • Direct the day-to-day functions of nursing assistants and ensure nursing personnel comply with facility policies, procedures, job descriptions, and applicable regulations.
  • Plan shift services, programs, activities, staffing, work assignments, and nursing service operations.
  • Review and recommend revisions to departmental policies, procedure manuals, job descriptions, and nursing procedures.
  • Prepare written and oral reports, administrative records, medical forms, evaluations, studies, charting, census reports, charge slips, and incident reports.
  • Coordinate nursing services with resident services and other departments to maintain each resident's total care regimen.
  • Participate in quality assurance programs, facility surveys, discharge planning, and resident care plan development and revision.
  • Admit, transfer, and discharge residents; complete related documentation; arrange transportation; and escort discharged residents as necessary.
  • Receive telephone orders from physicians, transcribe orders, maintain resident charts, document nursing care and resident responses, and report discrepancies.
  • Prepare and administer physician-ordered medications and treatments; maintain medication supplies and narcotic records; and report discrepancies.
  • Participate in employee performance evaluations, determine staffing needs, report absences, review workforce needs, and make recommendations concerning personnel actions.
  • Provide leadership to assigned nursing personnel, conduct rounds, monitor work assignments, and address problem areas, complaints, and grievances.
  • Inform personnel of admissions, prepare rooms, orient residents and families, make physician rounds, coordinate diagnostic and therapeutic services, and consult physicians regarding resident care.
  • Review charts and care plans; administer or oversee treatments; monitor resident condition; obtain specimens; record vital signs; and provide catheterization, tube feedings, suction, wound dressing, colostomy, drainage-bag care, range-of-motion exercises, and other professional nursing services.
  • Notify physicians and next of kin of accidents, incidents, and changes in resident condition; follow post-mortem procedures and inform families of resident deaths.
  • Develop, plan, conduct, schedule, and participate in in-service, orientation, continuing education, safety, infection-control, and advance-directive training.
  • Monitor safety, sanitation, hand hygiene, infection-control, protective-equipment, fire-safety, and disaster-preparedness compliance.
  • Maintain adequate supplies, medications, equipment, safety data sheets, and safe equipment-operation procedures.
  • Review resident care plans and ensure certified nursing assistants follow them; assist with MDS, RAPs, and triggers.
  • Report suspected fraud, hazardous conditions, occupational exposures, unauthorized access, and confidentiality or privacy violations.

PACS Group has appointed Carey P. Hendrickson as chief financial officer, effective 27 April 2026. Hendrickson brings nearly four decades of financial leadership across healthcare, senior living and media sectors. Most recently, Hendrickson served as CFO of U.S. Physical Therapy, overseeing 779 outpatient clinics across 44 states. He previously held CFO roles at Capital Senior Living Corporation, managing 128 communities across 23 states, and Belo Corp. PACS Group has announced that co-founder and chief financial officer Mark Hancock will retire on 30 June 2026. He will continue serving on the company's board of directors as vice chairman. Hancock co-founded PACS in 2013 with Jason Murray, starting with two post-acute care facilities in San Diego. Under his leadership, the company expanded to 323 facilities across 17 states, serving over 31,700 patients daily and generating $5.29 billion in revenue in 2025, representing 29.3% year-on-year growth. PACS Group, a leading post-acute healthcare platform, has appointed Dr Patrick Conway to its board of directors. Dr Conway currently serves as CEO of Optum, UnitedHealth Group's health services division with revenues exceeding $200 billion.
