- Graduate of an accredited Physical Therapist Assistant program.
- Associate degree as a Physical Therapist Assistant.
- Current Physical Therapist Assistant license in the state of practice.
- Strong communication, teamwork, and clinical problem-solving skills.
- Ability to deliver patient-centered care in a Skilled Nursing Facility setting.
- Deliver physical therapy treatments according to established plans of care under the supervision of a Licensed Physical Therapist.
- Help patients improve mobility, strength, balance, function, and independence.
- Communicate patient progress and concerns with the supervising Physical Therapist and interdisciplinary care team.
- Collaborate with physicians, nursing staff, caregivers, and families to support patient outcomes.
- Participate in care planning and interdisciplinary team meetings as appropriate.
- Provide education and training to patients, families, and facility staff.
- Complete timely electronic documentation, treatment notes, and daily billing activities.
- Assist with discharge planning and continuity of care.
- Support restorative and functional maintenance programs within the facility.
- Maintain productivity and documentation standards while delivering patient-centered care.
