Requirements
  • An associate's degree in human services and two years of full-time work experience providing care or education to vulnerable adults or children, or a bachelor's degree in human services and one year of full-time work experience providing care or education to vulnerable adults or children.
  • A car, auto insurance, and a valid driver's license.
Responsibilities
  • Provide treatment services that promote recovery and community integration for adults with serious persistent mental illnesses and co-occurring substance use disorders.
  • Oversee a group home with four residents and provide supervision and clinical direction to staff.
  • Coordinate client care, including establishing quarterly case conferences with case managers, treatment teams, family members, or other persons the client wishes to have present.
  • Contact residents' family members, therapists, or case managers as needed.
  • Develop, implement, and review quarterly the Individual Service Plan, Individual Abuse Prevention Plan, Emergency Service Plan, and relevant behavior plans in consultation with the Program Manager or Program Director.
  • Coordinate and schedule client medical appointments, complete referrals, and arrange transportation.
  • Identify individuals prone to behavior emergencies and develop additional programming for impulse control, dangerous behaviors, and self-injurious behaviors.
  • Assist the Program Director and Program Manager with client admission and discharge processes, including gathering clinical information, developing treatment plans, and connecting clients with community resources upon discharge.
  • Orient new residents to Residential Support Services.
  • Defuse and de-escalate potential behavior emergencies and use crisis assessment tools for suicidal and aggressive behavior.
  • Assist clients with independent living skills, including hygiene checks, housekeeping, and budgeting.
  • Assist residents with budgeting and coordinate budgets with representative payees.
  • Aid residents in completing financial forms, including applying for benefits upon admission.
  • Ensure facility security and general supervision of residents, including creating staff schedules.
  • Secure coverage for missed shifts, including covering shifts when no other coverage is secured.
  • Lead weekly house staff meetings and distribute meeting minutes to program leadership and house staff.
  • Administer medications according to policy and procedures under the facility nurse's supervision.
  • Ensure completion of monthly pharmacy medication orders and ensure clients receive ordered medications.
  • Update medication sheets when medication changes occur.
  • Produce and distribute incident reports during the shift of the relevant event as needed.
  • Attend agency meetings and participate in clinically oriented groups as requested.
  • Assist with licensing requirements and inspections, including posting emergency procedures.
  • Perform client record audits for compliance with Department of Human Services and county regulations.
  • Ensure fire and tornado drills are completed monthly.
  • Document financial expenditures in QuickBooks and manage petty cash and budgets.
  • Rotate emergency shift coverage pager.
  • Perform routine cleaning and light house maintenance and request maintenance assistance as needed.
  • Prepare meals, serve, and clean up according to Health Department standards.
  • Oversee planning and coordination of recreational and social activities.
  • Oversee planning of holiday activities and special events.
  • Transport clients in the community as needed.
