Requirements
  • Master's degree in advanced nursing; preferably in geriatrics
  • Must be a licensed Nurse Practitioner with prescriptive authority in the applicable work state
  • Must possess, or be eligible to obtain, Florida Autonomous Practice certification for Nurse Practitioners (candidates not currently approved must demonstrate the ability and willingness to meet Florida requirements for autonomous practice within the first year of employment)
  • Minimum of five years' experience working as a Nurse Practitioner in a primary care, urgent care, or other medical office setting, particularly within geriatrics
  • Experience in continuing care, skilled nursing care, long-term care, and/or in an assisted living environment
  • Must possess and maintain a valid state driver’s license
  • Compliance with Florida's Care Provider Background Screening process
Responsibilities
  • Review the medical history and disease trajectory of the referred resident, and make palliative care recommendations back to the referring physician or Nurse Practitioner.
  • Counsel the resident and their family in decision making regarding the treatment options for chronic conditions, symptom management, serious illness, or end of life care.
