Requirements
  • Registered Nurse licensed by the Virginia Board of Nursing.
  • Bachelor’s degree in Nursing from an accredited program by the National League for Nursing.
  • Demonstrated ability to supervise and direct professional and administrative personnel.
  • Ability to market and deal tactfully with customers and the community.
  • Must be a licensed driver with an automobile that is insured in accordance with state or organizational requirements and in good working order.
  • Knowledge of business management, governmental regulations, and CHAP standards.
  • Ability to access patients’ homes that may not be routinely wheelchair accessible.
  • Hearing, eyesight, and physical dexterity sufficient to perform physical assessments and patient care.
  • Ability to use computers, multi-line telephones, calculators, copiers, and other office and medical equipment.
Responsibilities
  • Coordinate and oversee direct and indirect patient services provided by clinical personnel.
  • Provide supervision and guidance to clinical staff to ensure service quality and maintenance of standards.
  • Perform chart audits.
  • Gather data for QAPI and participate in QAPI activities.
  • Appraise clinical staff performance during probationary and annual periods and provide teaching and counseling to enhance service delivery and the professional development of nursing and health aide staff.
  • Identify and evaluate patient needs and the appropriateness of home care by reviewing the initial intake screening form before assigning direct care staff.
  • Assist with the orientation of new staff and provide in-services for current staff.
  • See patients in the home setting as needed.
  • Maintain adequate and appropriate medical supplies.
  • Ensure Medicare, state, and ACHC regulations are followed by clinical staff.
  • Assist the Director of Nursing with requested responsibilities.
  • Pursue ongoing professional development through formal education, workshops, conferences, professional meetings, research, and reading.
  • Travel to patients’ homes when supervision of care is scheduled.
  • Bend, stoop, reach, and move patient weight up to 250 pounds and lift or carry up to 30 pounds when required.
