Requirements
- An active Arizona Licensed Practical Nurse license in good standing is required.
- A valid driver's license and reliable transportation are required.
- The candidate must be able to obtain or maintain a Level One Fingerprint Clearance Card, CPR and First Aid certification, and Article 9 certification.
- The candidate must demonstrate strong clinical judgment and communication skills.
- The candidate must be able to work independently and as part of a team.
- The candidate must be dependable and able to maintain consistent attendance.
- The candidate must be available for one weekend swing shift per month.
Responsibilities
- Provide nursing care and health monitoring for residents in group home settings.
- Administer medications and ensure accurate medication administration record documentation.
- Complete routine assessments and report changes in condition.
- Support direct support professionals with health-related tasks and training as needed.
- Assist with activities of daily living alongside direct support professionals.
- Maintain compliance with Developmental Disabilities Division, Home and Community-Based Services, and agency nursing standards.
- Communicate with the Licensed Practical Nurse Coordinator, managers, and clinical leadership.
- Assist with care plans, follow-up instructions, and physician orders.
- Ensure a safe, clean, and supportive environment for all residents.
- Participate in monthly nursing reviews and audits as required.
