Requirements
  • Bachelor's Degree in Nursing or Master's Degree in Social Work
  • 5+ years of Case Management experience
  • 2+ years of leadership experience
  • RN license or LCSW or MSW
  • Inpatient Case Management experience in an Acute Care Hospital setting is preferred but not required
Responsibilities
  • Manage designated aspects of case coordination, prior authorization, clinical documentation, and concurrent review programs to ensure maximal reimbursement, prevent financial loss to the hospital, and minimize patient liability
  • Serve as an escalation point for complex discharges
  • Manage staff including hiring, training, assigning work, managing and evaluating performance, and professional development planning
  • Ensure productivity and actions of the group meet and support the overall operational goals of the department as established by department leadership
  • Develop and adhere to departmental staffing, revenue and/or expense budgets
  • Respond to changes in the business which may affect the ability to achieve the budgets goals
  • Adjust programs to support the needs of the target patient population
  • Ensure compliance with TJC, state and other regulatory agencies
