Requirements
  • Current Registered Nurse licensure in good standing in the state or states of practice.
  • Ability to collaborate with nurses, social workers, chaplains, volunteers, and Certified Nursing Assistant leadership.
  • Ability to work independently with phone, email, written communication, meetings with healthcare professionals, and hospice rules and regulations.
  • Time-management skills to manage visits, documentation, meetings, scheduled visits, required meetings, urgent needs, prioritization, and triage of multiple concerns.
  • Ability to learn an electronic medical records system.
  • Willingness to drive with reliable transportation, a valid driver's license, and auto insurance to travel across the service area.
Responsibilities
  • Provide care and services for a group of hospice patients at their places of residence, including private homes, skilled nursing facilities, and assisted living facilities.
  • Provide expertise regarding psychosocial assessments, interventions, and community resources as a contributing member of the interdisciplinary team.
  • Work in hospice admissions and complete required visits.
  • Work one-on-one with patients while receiving support from the patient care team.

Interdisciplinary teams coordinate nursing, medical, social-work, spiritual, aide, and bereavement support. Care is delivered in homes and other residential settings with an emphasis on comfort, dignity, and quality of life. Brighton serves patients who have elected comfort-focused end-of-life care and works with caregivers and referring clinicians through an interdisciplinary model combining symptom management, psychosocial support, spiritual care, and bereavement services. Services are coordinated around an individualized plan of care and changing symptoms.
