Requirements
  • Graduate of an accredited Nursing program.
  • Hold a current Texas nursing license or a Nursing Licensure Compact license.
  • Hold Basic Life Support certification accredited by the American Heart Association or American Red Cross.
Responsibilities
  • Lead a multidisciplinary team in developing, refining, updating, and communicating the plan of care for a defined patient population.
  • Implement clinical pathways, including addressing patients' and families' educational needs and monitoring and updating care plans when deviations occur.
  • Partner with community liaisons to proactively manage care transitions between care settings and communicate with ongoing providers.
  • Collaborate with Social Work, Utilization Review, Clinical Documentation Specialists, and other necessary parties to maximize financial and care resources for patients and families.
  • Coordinate care with the care team to maximize patient throughput and minimize length of stay.
  • Round on patients daily to monitor and ensure effective patient and family preparation for discharge and completion of learning and discharge objectives.
  • Communicate with healthcare providers, patients, families, hospital personnel, and visitors, including adolescents, adults, and older adults.
  • Act as a liaison with departments and ancillary services to ensure pending orders and results are received promptly.
  • Ensure follow-up appointments are scheduled before patient discharge and that care-coordination discharge instructions are documented and understood by the patient and family or caregiver.
  • Collaborate with physicians, the unit manager, unit staff, and the interdisciplinary team to ensure safe and timely discharge.
  • Perform other duties as assigned.
Desired Qualifications
  • Bachelor's Degree in Nursing.
  • Two years of experience as a registered nurse.
  • Experience in an acute care hospital setting.
