Requirements
  • A master’s or doctoral degree as a board-certified Nurse Practitioner or Physician Assistant is required.
  • A current state license as a Nurse Practitioner or Physician Assistant is required.
  • Reliable transportation, a valid driver’s license, and valid automobile insurance coverage are required.
  • Applicants should demonstrate nursing skills consistent with accepted nursing standards.
  • Applicants should be able to assess and respond to the needs of patients, families, and caregivers in post-acute care settings.
  • Applicants should demonstrate skill in nursing practice, the ability to cope with family emotional stress, and tolerance of individual lifestyles.
  • Applicants should be able to handle multiple priorities, documentation requirements, and deadlines.
Responsibilities
  • Provide primary and consultative care to geriatric and medically complex patients in their homes or care settings.
  • Conduct comprehensive medical evaluations.
  • Develop and manage individualized treatment plans.
  • Prescribe and monitor medications.
  • Coordinate care with physicians, caregivers, and interdisciplinary team members within a collaborative practice framework.
  • Serve as a key clinical decision-maker.
  • Help patients and families understand diagnoses, manage chronic illness, and avoid unnecessary hospitalizations.
Desired Qualifications
  • At least two years of clinical experience in post-acute care or as a Nurse Practitioner or Physician Assistant.
  • Adult, Family, or Gerontology certification.
  • Experience with geriatric patients.

Optimal Care provides Medicare-certified home health services for seniors, guided by a nurse practitioner-led care model. A nurse practitioner visits patients at home and develops individualized care plans that cover behavioral health, dementia, geriatric, diabetic, wound, and orthopedic care. This in-home, NP-driven approach offers a broad suite of geriatric-focused programs rather than only post-surgical or rehabilitation services. Its goal is to keep seniors safe and healthy at home by coordinating comprehensive, specialty care in their living environments.
