  • A current Licensed Practical Nurse license in the State of Kentucky is required.
  • Current CPR certification is required, or the ability to obtain it within the required timeframe.
  • Strong knowledge of nursing principles, medication administration, and resident care practices is required.
  • The ability to prioritize multiple responsibilities in a fast-paced healthcare environment is required.
  • The position requires meeting physical demands including lifting up to 50 pounds, prolonged standing and walking, bending, pushing, pulling, and assisting residents with mobility and daily care needs.
  • The role requires the ability to work independently while collaborating effectively with an interdisciplinary healthcare team.
  • The role requires communication, interpersonal, organizational, and compassionate resident-centered care abilities.
Responsibilities
  • Administer medications, treatments, and nursing interventions in accordance with physician orders and facility policies.
  • Monitor resident conditions, assess changes in health status, and promptly communicate concerns to the Registered Nurse or physician.
  • Record vital signs and maintain accurate, timely, and compliant clinical documentation.
  • Assist residents with activities of daily living while promoting dignity, comfort, and independence.
  • Collaborate with physicians, Registered Nurses, Certified Nursing Assistants, therapists, and the interdisciplinary care team to implement individualized care plans.
  • Provide education, emotional support, and compassionate communication to residents and their families.
  • Ensure compliance with infection prevention, resident safety, and regulatory standards.
  • Respond appropriately to changes in resident condition and emergency situations.
  • Participate in quality improvement initiatives, care conferences, and ongoing professional development.
  • Foster a positive, respectful, and collaborative environment focused on delivering exceptional resident care.
Desired Qualifications
  • One or more years of experience in skilled nursing, long-term care, rehabilitation, assisted living, or post-acute care.
  • Experience caring for older adults with complex medical needs.
  • Knowledge of electronic medical records and nursing documentation systems.
  • Experience with medication administration, wound care, restorative nursing, or dementia care.
  • Strong clinical judgment, time management, and critical thinking skills.
