Requirements
- A current Licensed Practical Nurse license in the State of Kentucky is required.
- Current CPR certification is required, or the ability to obtain it within the required timeframe.
- Strong knowledge of nursing principles, medication administration, and resident care practices is required.
- The ability to prioritize multiple responsibilities in a fast-paced healthcare environment is required.
- The position requires meeting physical demands including lifting up to 50 pounds, prolonged standing and walking, bending, pushing, pulling, and assisting residents with mobility and daily care needs.
- The role requires the ability to work independently while collaborating effectively with an interdisciplinary healthcare team.
- The role requires communication, interpersonal, organizational, and compassionate resident-centered care abilities.
Responsibilities
- Administer medications, treatments, and nursing interventions in accordance with physician orders and facility policies.
- Monitor resident conditions, assess changes in health status, and promptly communicate concerns to the Registered Nurse or physician.
- Record vital signs and maintain accurate, timely, and compliant clinical documentation.
- Assist residents with activities of daily living while promoting dignity, comfort, and independence.
- Collaborate with physicians, Registered Nurses, Certified Nursing Assistants, therapists, and the interdisciplinary care team to implement individualized care plans.
- Provide education, emotional support, and compassionate communication to residents and their families.
- Ensure compliance with infection prevention, resident safety, and regulatory standards.
- Respond appropriately to changes in resident condition and emergency situations.
- Participate in quality improvement initiatives, care conferences, and ongoing professional development.
- Foster a positive, respectful, and collaborative environment focused on delivering exceptional resident care.
Desired Qualifications
- One or more years of experience in skilled nursing, long-term care, rehabilitation, assisted living, or post-acute care.
- Experience caring for older adults with complex medical needs.
- Knowledge of electronic medical records and nursing documentation systems.
- Experience with medication administration, wound care, restorative nursing, or dementia care.
- Strong clinical judgment, time management, and critical thinking skills.
