Join our team and make a positive impact on the lives of others! We are looking for a Care Manager for our Massachusetts team.
This role is field base position and will require you to travel at least 4 times a week. There will also be potential travel to our FreedomCare hub closest to you.
Department & Position Overview:
The Care Manager will perform in home quality visits in the home of the Adult Foster Care member to determine quality of care matches the members’ needs. You will also conduct monthly visits with members who are On Care with FreedomCare to ensure they are address any changes in their condition and provide additional support and training to the Caregiver.Adult Foster Care is a caregiving arrangement in which a patient lives with a principal caregiver who provides daily support based on the patient’s daily care needs. The goal of this service is to provide necessary care while fostering and emphasizing the patient’s independence in a home environment that will provide them with a range of care options as their needs evolve. The service is designed to provide options for alternative long-term care to persons who meet nursing facility-level care needs while preserving the dignity, self-respect, and privacy of the patient.
Every Day You Will:
- Provide support to the caregiver and member, and review services according to the scope of Adult Foster Care and the members Plan of Care.
- Respect client and employee rights and privacy, ensure the security of protected information, practices in an ethical manner and is compliant to the agency’s compliance program and privacy policy.
- Follow the FreedomCare’s clinical policy and procedures.
- Assesses and regularly reassesses the client’s needs and his/her response to care, treatment, their qualified setting.
- Perform home inspections annually to ensure the qualified setting is a safe environment for the caregiver & member. Will review and document the fire & safety procedures for the qualifies setting.
- Provide education and training to caregivers through in-person demonstrations and through our LMS platform. They will guide the Caregiver to ensure they can meet the needs of the member as described in the plan of care.
- Reviewing AFC Caregiver Logs (Daily Notes) at minimum of every 30 days.
- Develops implements, evaluates, revises, and provides input to the client’s plan of care to include the physician’s plan of treatment, and problem list.
- Completes all client documentation, including written records, reports, and notes in accordance with agency policy and procedure following client visits or contacts in an accurate, timely, complete, concise, and legible manner.
- Actively coordinates the care of the client through communication and written documentation to the physician, RN and Program Director, other disciplines involved in the client’s care.
- Interprets the philosophy and objectives of the agency to clients, families, and others.
- Performs other job-related duties as may be required or directed by the supervisor.
Ideal Candidate Will Possess:
- Bachelor’s degree, a social worker license from the Massachusetts Board of Registration in Social Work, and at least two years of recent experience working with elders or adults with disabilities (OR) Bachelor’s degree and two years of clinical experience in the care of elders or people with disabilities.
- Experience with CRM (Salesforce preferred)
- Valid driver’s license with an insured vehicle for travel within the state of Massachusetts
- Ability to travel in the field to accompany patients to appointments and meet patients in person when needed
- Strong working knowledge of local community resources
- Demonstrated ability to work with data reporting, documentation, and outcomes
- Strong communication and assessment skills; the ability to relate to patients, their families, and community care providers, along with ability to handle rapidly changing crisis situations
- Ability to manage high volume caseloads
- Able to express empathy and compassion for the underserved
Nice-to-Haves:
- Bilingual Spanish strongly preferred
- Master’s degree in Social Work
