Requirements
  • Bachelor’s degree
  • Social worker license from the Massachusetts Board of Registration in Social Work
  • 2 years of recent experience working with elders or adults with disabilities
  • OR Bachelor’s degree and 2 years of clinical experience in the care of elders or people with disabilities
  • Experience with CRM (Salesforce preferred)
  • Valid driver’s license with an insured vehicle for travel within the state of Massachusetts
  • Ability to travel in the field
  • Strong working knowledge of local community resources
  • Strong communication and assessment skills
  • Ability to manage high volume caseloads
  • Express empathy and compassion for the underserved
Responsibilities
  • Perform in-home quality visits
  • Conduct monthly visits with members
  • Provide support to caregivers and members
  • Respect client and employee rights and privacy
  • Follow clinical policies and procedures
  • Assess client's needs and response to care
  • Perform home inspections
  • Provide education and training to caregivers
  • Review AFC Caregiver Logs
  • Develop, implement, evaluate, and revise client's plan of care
  • Complete client documentation
  • Coordinate care of the client
  • Interpret agency's philosophy and objectives
  • Perform other job-related duties
