Requirements
  • A current, unencumbered Ohio license to practice as a Registered Nurse is required.
  • One year of experience in acute care or medical-surgical nursing is required.
  • Proficiency with Windows, Microsoft Office including Word, Excel, and PowerPoint, and the internet is required.
  • The ability to read, write, speak, and understand English is required.
  • A valid driver's license and acceptable driving record are required, along with insurability under professional liability and crime coverage policies specified by the insurance carrier's underwriting standards.
  • The role requires the ability to lift, transfer, push, or pull patients or equipment and supplies weighing up to 50 pounds.
  • Weekend, evening, and holiday work is required as scheduled.
  • On-call availability is required 24/7 for emergencies.
  • The role may involve exposure to falls, equipment burns, odors, cuts, infectious waste, disease conditions, AIDS, and Hepatitis B viruses, with bloodborne-disease exposure Risk Category I.
Responsibilities
  • Provide ongoing assessment and nursing care to patients in homes and healthcare facilities.
  • Evaluate patient needs accurately and develop and implement comprehensive care plans addressing current needs.
  • Identify when home health patients need or qualify for additional services and notify the appropriate team member or team leader.
  • Follow current nursing professional standards of practice and agency-approved home health and hospice protocols.
  • Provide quality pain and symptom management for patient comfort.
  • Communicate with other healthcare providers about changes in patient condition and needs.
  • Assess patient and caregiver needs and provide education when necessary.
  • Provide high-quality customer service to internal and external customers.
  • Oversee implementation of patients' plans of care and evaluate their effectiveness on an ongoing basis.
  • Prepare for interdisciplinary group meetings by reviewing care plans, reconciling medications, and projecting patient and family needs when applicable.
  • Audit and maintain current, accurate patient records using laptop computers and paper or chart documentation in accordance with company policies and federal, state, and local guidelines.
  • Participate in quality improvement projects as needed.
  • Maintain knowledge of requirements from regulatory agencies, accrediting bodies, and third-party payers.
  • Coach and influence LPN Field Staff and Home Health Aides regarding their performance expectations.
  • Foster a positive work environment that attracts, retains, and motivates employees.
  • Interview, select, and orient new employees.
  • Define, establish, and promote performance expectations so staff understand and demonstrate high-quality, consistent performance.
  • Evaluate staff job performance in accordance with company policies and procedures and provide developmental guidance for improved performance and growth.
  • Monitor staff accountability under organizational requirements prescribed by the Employee Handbook.
  • Participate in developing, planning, scheduling, conducting, and monitoring in-service training, on-the-job training, and orientation programs for departmental staff.
  • Help ensure reasonable and sufficient staffing coverage when a direct report is absent or a position is vacant.
  • Respond promptly and thoughtfully to questions, requests, and work product from departmental staff submitted for review and approval.

  • Experience in a Medicare-certified home health agency.
  • Proficiency with electronic medical records.
