Requirements
  • Bachelor’s degree in Nursing from an accredited institution.
  • Active Registered Nurse license in South Carolina or compact state.
  • Minimum of 5 years RN nursing experience.
  • Certified Diabetes Care and Education Specialist (CDCES) certification required within one year of hire.
  • Registered nurses with no CDCES must obtain 15 diabetes related CEU's within 30 days from date of hire.
  • Bachelor's degree in a nutrition-focused field.
  • Registered Dietitian credential from the Commission on Dietetic Registration.
  • Minimum of 5 years clinical experience.
  • Certified Diabetes Care and Education Specialist (CDCES) required within one year.
Responsibilities
  • Coordinate and provide age specific and developmentally appropriate care for pediatric diabetes patients across Ambulatory spaces.
  • Serve as the primary educator and leader for diabetic care coordination.
  • Support organizational efforts as needed that include access, quality improvement, literature review, patient advocacy, and outpatient staff development.
  • Establish educational programs required to meet identified patient, staff, and organizational goals.
  • Promote a positive, motivating environment to support staff retention.
  • Guide staff to appropriately manage conflict, ongoing problems, and department issues.
  • Delegate appropriately with defined expectations.
  • Verify accurate documentation in electronic medical record to enhance revenue collections.
  • Advocate for patients and their significant others, coworkers, the community, and the nursing or dietitian profession.
  • Champions collaborative practice with coworkers and other disciplines and departments.
  • Demonstrates compassion to internal and external customers.
  • Integrates theoretical knowledge base and clinical expertise in decision making.
  • Assumes responsibility for meeting regulatory and other mandatory requirements.
  • Assumes responsibility for personal development plan and education/certification related to clinical specialty area.
  • Recognize learning needs and obtain guidance to acquire appropriate education and/or training.
  • Proactively contribute recommendations to improve patient care and clinic operations.
  • Coordinate and manage care by applying analytical reasoning, reflection, rational problem-solving skills, using evidence-based information and clinical judgment.
  • Assist with the development of personal objectives.
  • Evaluate practice through discussion with appropriate staff and self-evaluation.
  • Accept and utilize constructive feedback to improve performance.
  • Orient and train new staff in assigned areas.
  • Provide timely written and verbal feedback.
  • Demonstrate effective use of computer-based information for decision support, continuous quality improvement and professional development.
  • Assists with message management as scope of practice requires.
  • Maintain efficiency with call response time and patient requests.
  • Monitor in basket messaging and communicate all relevant information required to coordinate care.
  • Complete and coordinate all relevant tests and procedures.
  • Build collaborative relationships with external agencies (ex. School nurses, DME companies) to promote the health of our diabetic patients.
