Requirements

Responsibilities
  • Work as part of a multidisciplinary team and collaborate with other professionals to meet the emotional, physical, social, and medical needs of participants
  • Deliver care in multiple settings: on-site clinic, participant homes, partner skilled nursing facilities
  • Participate in a rotating after-hours on-call schedule to support the on-call nurse
  • Build strong relationships with participants and their families through education and support
  • Practice in a flexible care model that adapts to participant needs rather than rigid systems

  • Experience in primary care or geriatric care (preferred)
  • An inspirational, motivational, and creative approach aligned with the PACE model of care

PACE North provides an all-inclusive care program for adults 55 and older in northern Michigan, delivering nursing-home-level care in participants’ homes. A capitation funding model from Medicare, Medicaid, and private pay supports coordinated services, while an Interdisciplinary Team builds individualized care plans that include medical care, medications, home care, therapy, transportation, meals, and social activities, often centered at a Day Center. Unlike fee-for-service models,PACE North offers a single, integrated approach across seven counties to manage medical, social, and functional needs in one program. The goal is to help seniors live safely and independently at home and prevent unnecessary nursing home placement.
