Requirements
  • A bachelor's degree or higher from an accredited school of nursing.
  • Three years of direct clinical nursing patient-care experience in an acute-care or specialty setting.
  • Leadership, charge, or coordinator experience.
  • A Texas Registered Nurse license, with a permanent Texas license required within 60 days if establishing Texas residency.
  • Basic Life Support certification or instructor status through the American Heart Association.
  • Magnet-ANCC recognized certification within one year.
  • Proficiency in speaking, reading, and writing English sufficient to perform essential job functions, especially those affecting patient or employee safety or security.
  • Strong psychosocial assessment and clinical skills.
  • Knowledge of community resources and the judgment and ability to select and coordinate available resources, including referrals to regulatory agencies.
  • Proficiency in the Leadership, Art and Science of Nursing at the Proficient level according to Benner's model of clinical practice.
  • Ability to work independently with organization, time-management, and prioritization skills.
Responsibilities
  • Assist patients and families in navigating the health care system within hospital programs.
  • Standardize and improve care efficiency for patients in the assigned service line or unit across the continuum of care.
  • Model patient-centered care and provide high-level contributions through direct patient interaction, within the unit and service line, and across the organization.
  • Assist management with coordinating program education and supporting growth of the service-line program.
  • Lead unit and shared-governance activities and assess service-line or unit practice and improvement needs.
  • Serve as a clinical resource for service-line program and hospital staff and evaluate direct costs and patient outcomes.
  • Develop relationships with physicians and health care providers to support effective communication in patient care.
  • Serve as a liaison among ancillary departments, clinicians, physicians, and patients to ensure efficient patient flow.
  • Provide advanced assessment consultation to interprofessional team members and develop educational information for nursing units.
  • Teach critical thinking by explaining decision rationales and coaching and critiquing colleagues' thought processes in complex situations.
  • Advocate for patients in preventing and resolving ethical issues and facilitate interventions for patient and family spiritual distress.
  • Mentor and coach staff in effective verbal, nonverbal, and written communication, including active listening, and facilitate teamwork.
  • Coordinate patient care and development within the population-specific program through personalized interaction.
  • Educate patients and families on disease-specific guidelines and treatment options, and facilitate program introduction, resource referrals, and counseling.
  • Develop educational resources and programs related to program services, teach nursing-care principles and advanced technical knowledge, and provide formal education to groups outside the unit.
  • Lead performance-improvement activities related to patient safety, identify clinical priorities, compare outcomes with national benchmarks, and recommend improvements.
  • Provide clinical expertise for complex or highly specialized patient-care situations and lead interprofessional teams in implementing plans of care.
  • Contribute to service-line and hospital standards and guidelines and implement and monitor initiatives that improve core-measure compliance.
  • Perform data-management activities including data abstraction, analysis, presentation, collection, and submission to appropriate databases.
  • Implement department strategies supporting financial targets through timely documentation, reduced length of stay, optimized efficiency, and related activities.
  • Participate in decisions affecting operational and capital needs.
  • Lead practice changes and develop, facilitate, and participate in educational programs for health care providers, patients, families, and community groups.
  • Attend workshops and educational programs, monitor professional developments and trends, submit abstracts to national conferences, and incorporate evidence-based practices into shared governance and leadership.
  • Coach staff to expand their knowledge, abilities, skills, and attitudes, and complete and update the individual development plan.
  • Work evening shifts, wear required scrubs and business-professional attire, participate in on-call coverage during emergencies, and travel within the Houston metropolitan area as required.
