Requirements
  • Graduated from an accredited nursing program within the past 12 months.
  • Obtain a nursing license and registration within the timeline defined by state regulations or within 90 days of hire, whichever occurs first.
  • Hold a current appropriate state Temporary Permit.
  • Meet the practice requirements in the state of employment.
  • Maintain all required credentials or certifications in active and valid status, including timely renewal.
  • Handle demanding situations, multitask, and remain flexible to changing needs.
  • Maintain regular and punctual attendance.
  • Perform the physical demands of direct patient care, including heavy lifting and moving patients, assisting with ambulation, prolonged standing, bending, and moving equipment with assistance.
  • Work in a healthcare environment with potential exposure to infectious and contagious diseases or materials.
  • Take the Ishihara Color Blindness test as a condition of employment.
Responsibilities
  • Direct Patient Care Technicians in the safe and effective delivery of chronic hemodialysis therapy in compliance with facility policies and applicable regulations.
  • Delegate tasks to direct patient-care staff, including Licensed Vocational Nurses or Licensed Practical Nurses, Patient Care Technicians, and Dialysis Assistants.
  • Assess daily patient-care needs and develop appropriate patient-care assignments.
  • Monitor patient-care staff for appropriate techniques and adherence to facility policies and procedures.
  • Participate in staff training, new-staff orientation, and required staff meetings.
  • Provide patients and families with education about End Stage Renal Disease, renal disease, vascular access, dialysis therapy, diet, fluid, medications, laboratory values, treatment compliance, transplant options, and dialysis modalities.
  • Provide safe and effective care to patients with End Stage Renal Disease receiving hemodialysis.
  • Implement treatment prescriptions, including sodium modeling and ultrafiltration modeling when appropriate.
  • Assess patient responses to hemodialysis, adjust treatment plans as indicated by the prescribing physician, and communicate concerns to the Team Leader or physician.
  • Identify and communicate patient-related issues to the Team Leader or physician.
  • Initiate initial and annual nursing assessments and continuously evaluate and document patient-care needs.
  • Participate in the pre-evaluation, initiation, monitoring, termination, access homeostasis, and post-evaluation of hemodialysis treatment.
  • Intervene when patient adequacy status changes and troubleshoot access-flow issues identified by Online Clearance Monitoring or the Adequacy Monitoring Program.
  • Provide, supervise when applicable, and monitor hemodialysis access care.
  • Administer, monitor, and document patients’ responses to prescribed interdialytic transfusions and report adverse reactions appropriately.
  • Ensure accurate and complete Patient Care Technician documentation on Hemodialysis Treatment Sheets.
  • Review, transcribe, and enter physician laboratory orders into the Medical Information System.
  • Prepare laboratory requisitions and ensure correct tubes, collection, processing, packaging, and delivery of specimens according to applicable procedures and Department of Transportation requirements.
  • Follow up on missed or insufficient laboratory specimens and report abnormal or critical laboratory results to the appropriate physician.
  • Enforce company policies and applicable state and federal regulations.
  • Maintain safe, efficient, and effective shift operations.
  • Act as a resource for other staff members and meet routinely with the Clinical Manager regarding personnel and patient-care status.
  • Collaborate with physicians and other healthcare professionals to interpret, adjust, and coordinate patient care.
  • Assist patients with prescription refills according to company policies.
  • Enter physician orders and patient information into the Medical Information System in a timely and accurate manner.
  • Oversee patient-information documentation and maintain the facility drug list for required stock medications.
  • Maintain competency in emergency procedures, initiate cardiopulmonary resuscitation and emergency measures during cardiac or respiratory arrest, and ensure adequate emergency equipment is available.
  • Ensure appropriate vaccinations, immunizations, and annual tuberculosis testing.
  • Administer prescribed or algorithm-approved medications, document medical justification when indicated, and document the effectiveness of as-needed medications.
  • Maintain controlled-substance records as required by law.
  • Assist with coordinating patient transportation when necessary.
  • Maintain a clean, safe, and sanitary dialysis treatment environment.
  • Operate dialysis-related equipment safely and effectively.
  • Ensure patient stations, machines, and chairs are clean and appropriately arranged.
  • Address blood spills immediately according to bloodborne-pathogen control policies.
  • Implement patient admission, transfer, and discharge procedures.
  • Review and ensure completion of daily Hemodialysis Treatment Sheets and other patient-care documentation.
  • Follow opening and closing procedures and monitor that staff and patients have safely left the premises.
  • Initiate, document, and complete monthly Continuous Quality Improvement activities and reports.
  • Complete monthly nurses’ progress notes and patient care plans.
  • Maintain complete and accurate patient medical records.
  • Review transplant status and follow established procedures.
  • Complete long-term programs when due.
  • Complete initial and annual Nursing History and Assessment physicals.
  • Ensure completion of Annual Standing Order Reviews with physicians.
