PATH is seeking candidates passionate about helping others make a positive change in their lives to join our CalAIM Team as the Team Supervisor for PATH San Diego.
ABOUT PATH
Since its foundation in 1984, PATH (People Assisting The Homeless) has pioneered bold and effective approaches to assist people experiencing homelessness. PATH operates services throughout California, connecting our clients to a comprehensive continuum of homelessness prevention, street outreach, employment preparation and placement assistance, individualized case management, supportive services, interim housing, and permanent supportive housing.
POSITION SUMMARY
The Team Supervisor is responsible for the direct supervision of a team within a program. The team supervisor oversees and supports staff to provide quality services to meet the needs of the clients and programmatic outcomes/deliverables. Duties and responsibilities include upholding organizational and programmatic standards, providing individual supervision and team meetings, reviewing and approving timecards, and providing trainings. The team supervisor may also work directly with clients.
ABOUT THE JOB
- Direct supervision of all program staff, and the assignment of tasks and adjustment of ongoing assignments relative to programmatic needs.
- Ensure that services emphasize trauma informed care, client safety, and harm reduction interventions.
- Obtain SOAR certification and write SOAR Medical Summary reports for individual applications.
- Assist eligible individuals with filing SSI/SSDI applications upon enrollment
- Coordinate visits to medical doctors, mental health providers, and other specialists to obtain evidence for SSI/SSDI application.
- Assist in the screening, assessment, and enrollment of clients in a range of services.
- Ensure adherence to funding source contracts and performance standards; prepare written reports on program activities as assigned and as required by funding sources.
- Participation and/or presentation in local meetings with community partners. This includes coordinating and collaborating with community partners on client care.
- Presentation of regularly monitored data and program outcomes to the Program Manager.
- Ensure that all program data is accurate and entered into the required program documentation system as contractually required.
- In collaboration with program manager, conduct employee performance management including correction action planning of direct report staff.
- Approve timecards and leave requests.

PREFERRED QUALIFICATIONS
- Two (2) years of experience in healthcare, case management, homeless services or working with vulnerable populations.
MINIMUM QUALIFICATIONS
- One (1) years of experience in healthcare, case management, homeless services or working with vulnerable populations.
MANDATORY REQUIREMENTS
Regardless of qualifications, candidates must:
- Have employment eligibility verification
- Have or be able and willing to obtain CPR/First Aid training
- Successfully complete the following as a condition of hire:
- Tuberculosis Test
- Background Screening
- Drug Test
DRIVING
Driving is an essential function of this position. To meet the employment requirements for this role, all candidates must:
- Have reliable transportation
- A valid driver’s license
- Proof of insurance and ownership for personal vehicles used during work duties
- The ability to qualify for PATH’s insurance coverage

FLEXIBLE WORK
We are a California employer. Therefore, all regular and customary work must be performed within the state. Approval for work outside of the office, does not establish approval for work to be conducted outside of California.
