- Deliver personalized bedside education to patients and caregivers on intravenous therapy administration, care of central lines, and safety protocols
- Provide training on infusion devices and techniques, verifying readiness before discharge
- Educate home health agency staff when needed to ensure proper care delivery post-discharge
- Conduct initial patient assessments to confirm appropriateness for home or alternate site care and ensure alignment with payer requirements
- Collect and communicate clinical documentation necessary for treatment planning and reimbursement
- Document all patient interactions, education, coordination efforts, and care plan updates in a timely and accurate manner
- Serve as a clinical liaison between the hospital and the Company, promoting services, addressing questions, and fostering trust with patients and referral sources
- Advocate for the patient’s needs and ensure clear communication across the care team
- Maintain ongoing contact with patients during their hospitalization and early stages of home or alternate site therapy to support adherence and address challenges
- Monitor referral platforms and maintain communication threads with internal teams
- Coordinate and attend meetings with hospital personnel and internal departments as needed
- Support marketing and business development efforts by identifying new referral sources and participating in hospital-based outreach
