A Bachelor's Degree in a related human services or social services field.
A valid driver's license and vehicle are required to travel.

Responsibilities
Be available, as determined by rotation, on a 24-hour basis to intervene in crisis needs.
Work closely with State Mental Health Facility to plan services needed for high-rate users referred for discharge from inpatient care.
Work closely with the community support treatment team triage.
Maintain professional working relationships with all referral agencies.
Develop plans to refer, link, and place individuals with community-based treatment and other needed resources.
Assist individuals by referring, linking, and placing them with recreation, transportation, financial and medical entitlement, and vocational rehabilitation services as appropriate.
Coordinate inter- and intra-agency services.
Conduct outreach and case finding.
Determine eligibility and conduct comprehensive assessments of service needs.
Develop comprehensive master Individual Treatment Plans with appropriate housing facilities, with input from the client and/or family unit.
Review clients' Individual Treatment Plans regularly to assess effectiveness and the need for specific plan elements, with input from the client or family.
Monitor and evaluate all services provided to individual clients.
Locate appropriate housing facilities as needed.
Provide supportive counseling to individual clients or families as needed.
Provide problem-solving assistance to clients or families as needed.
Provide public education on services available and the needs of mentally ill individuals as opportunities arise or as assigned by the Vice President.
Participate in staff meetings scheduled by the supervisor.
Document all services provided in progress notes and perform DIS billings of services on a timely basis.
Maintain appropriate credentials and pursue internal or external training and development opportunities.
Perform other duties as assigned.

Two-three years of experience in a mental health setting.
Knowledge of languages other than English, particularly Spanish.

WORKING CONDITIONS AND PHYSICAL EFFORT:
Work is normally performed in a typical interior/office work environment.
Moderate physical activity. May require physical effort including lifting up to 10-25 pounds and some extended periods of standing or walking.
