- Graduate from an accredited School of Nursing program.
- Hold a registered nurse license.
- Be certified as a Nurse Practitioner through an accredited Nurse Practitioner Program.
- Have a Master's Degree in Nursing with a concentration and specialty track in Older Adult and/or Family Nurse Practitioner (FNP); other concentrations or specialty tracks may be considered based on experience and business need.
- For the Physician Assistant path, graduate from a program accredited by the Accreditation Review Commission on Education for the Physician Assistant (ARC-PA), take the Physician Assistant National Certifying Exam (PANCE), hold state licensure, and maintain National Commission on Certification of Physician Assistants (NCCPA) certification.
- Maintain an unrestricted professional license and Drug Enforcement Administration (DEA) registration in each state of practice, where required by state practice rules.
- Provide primary and urgent care services to nursing center residents in collaboration with other medical providers in the center.
- Collaborate with Center Leadership on center-specific goals and priorities.
- Collaborate with the Medical Director and/or Attending Physician for patient-specific medical care.
- Work collaboratively with Center Leadership to improve patient outcomes through high-quality health care and compassionate care.
- At least two years of experience working with adult or geriatric populations.
