Requirements
  • Provide direct nursing care to residents and supervise the day-to-day nursing activities performed by nursing assistants in accordance with applicable federal, state, and local standards, guidelines, and regulations.
  • Possess, at minimum, a nursing degree from an accredited college or university, or graduate from an approved Licensed Practical Nurse/ Licensed Vocational Nurse program.
  • Possess a current, unencumbered, active license to practice as a Licensed Practical Nurse/ Licensed Vocational Nurse in the state.
  • Read, write, speak, and understand English, including technical procedures and policy and procedure manuals.
  • Effectively present information, respond to questions from managers and employees, and communicate information concerning a resident’s condition.
  • Apply fractions, percentages, ratios, and proportions to practical situations.
  • Demonstrate the knowledge and skills necessary to provide care appropriate to the age-related needs of residents.
  • Make independent decisions when circumstances warrant such action.
  • Deal tactfully with personnel, residents, family members, visitors, government agencies, and the general public.
  • Be knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines pertaining to nursing care facilities.
  • Possess leadership and supervisory ability and the willingness to supervise other personnel.
  • Plan, organize, develop, implement, and interpret programs, goals, objectives, policies, and procedures necessary to provide quality care.
  • Seek out new methods and principles and incorporate them into existing nursing practices.
  • Meet the facility’s general health requirements, including a medical and physical examination.
  • Move intermittently throughout the workday and push, pull, move, and/or lift at least 25 pounds to a minimum height of 5 feet and distance of 50 feet.
  • Assist with evacuating residents during emergency situations when necessary.
Responsibilities
  • Direct the day-to-day functions of nursing assistants and ensure nursing personnel comply with facility policies, procedures, job descriptions, and applicable standards.
  • Review department policies, procedure manuals, and job descriptions and recommend revisions.
  • Plan shift services, programs, and activities with assigned nursing and support personnel.
  • Maintain the Nursing Service Procedures Manual and ensure compliance with its procedures.
  • Prepare written and oral reports and recommendations concerning shift activities.
  • Coordinate nursing services with other resident services to maintain each resident’s total care regimen.
  • Participate in the nursing service quality assurance program and facility surveys conducted by authorized government agencies.
  • Review and update resident discharge plans and assist with the nursing services portion of discharge planning.
  • Interpret department policies and procedures for personnel, residents, visitors, and government agencies.
  • Admit, transfer, and discharge residents, complete related forms and reports, and maintain census and administrative records.
  • Protect resident and facility information by safeguarding user credentials and protected health information and reporting unauthorized access attempts.
  • Complete and file recordkeeping forms and charts for resident admissions, transfers, and discharges.
  • Receive telephone orders from physicians, transcribe orders to charts and care records, and encourage physicians to review and sign treatment plans and orders.
  • Chart nursing notes and accident or incident reports accurately, descriptively, and according to established procedures.
  • Record diet changes, report discrepancies, complete transfer forms, and ensure required documentation is entered in resident medical records.
  • Prepare and administer physician-ordered medications and treatments after verifying resident identity.
  • Maintain medication, narcotic, supply, and equipment inventories; order prescribed medications and supplies; review medication cards; report discrepancies; and dispose of drugs and narcotics according to procedure.
  • Participate in employee performance evaluations, determine shift staffing requirements, and recommend employee dismissals or transfers to the Nurse Supervisor.
  • Report staffing shortages and absentee call-ins, evaluate the department workforce, and recommend staffing actions.
  • Develop work assignments, provide leadership to assigned nursing personnel, and make daily rounds to monitor completion of nursing assignments.
  • Ensure nursing assistants are enrolled in or have graduated from approved training programs and that nurse aide trainees receive direct supervision from a licensed nurse.
  • Meet regularly with shift nursing personnel to identify and correct problems and improve services.
  • Review personnel complaints and grievances and report or address them according to facility procedures.
  • Receive and give nursing reports at the beginning and end of shift duty hours.
  • Report occupational exposures, suspected fraud, and information-security violations according to facility policies.
  • Inform nursing personnel about new admissions, prepare rooms, greet and orient new residents and family members, and escort residents as needed.
  • Make physician rounds, arrange diagnostic and therapeutic services, consult physicians about resident care, and review resident charts for treatments, medications, and diets.
  • Implement nursing objectives and standards and verify that prescribed treatments are properly administered.
  • Evaluate residents’ physical and emotional status and ensure direct care follows resident care plans and wishes.
  • Notify physicians and next of kin about accidents, incidents, or changes in residents’ conditions.
  • Carry out restorative and rehabilitative programs, including self-help and care.
  • Administer professional nursing services including catheterization, tube feedings, suction, dressing and bandage changes, colostomy and drainage-bag care, blood collection, massage, range-of-motion exercises, and care for dying residents.
  • Use restraints according to established policies and procedures and obtain ordered sputum, urine, and other laboratory specimens.
  • Take and record temperature, pulse, respirations, blood pressure, and other required measurements and monitor seriously ill residents.
  • Check food brought by visitors against residents’ dietary allowances and report problems.
  • Check residents who are unable to call for help and meet with residents or family members as needed.
  • Arrange transportation for discharged residents, escort them to pickup areas, notify family members of deaths, contact funeral homes when requested, and follow post-mortem procedures.
  • Develop, plan, conduct, and schedule in-service training and orientation programs for nursing personnel.
  • Standardize work methods, assist with training on blood and body-fluid exposure tasks, and assist with clinical supervision for nurse aide trainees.
  • Attend outside training, annual facility in-service programs, advance-directive training, and continuing education programs.
  • Monitor assigned personnel for compliance with safety regulations, dress codes, hand hygiene, infection control, isolation, fire safety, disaster preparedness, and personal protective equipment procedures.
  • Maintain clean, safe, and sanitary nursing work areas, resident care rooms, and treatment areas.
  • Participate in developing and maintaining infection-control programs and procedures for communicable and infectious diseases.
  • Ensure appropriate safety equipment, mechanical lifts, hazardous-chemical documentation, and material safety data sheets are available and used properly.
  • Recommend unit equipment and supply needs, maintain adequate stock, implement safe equipment-operation procedures, and monitor efficient supply use.
  • Review resident care plans daily, report needed revisions, ensure nurses’ notes reflect care-plan compliance, and verify that certified nursing assistants follow care plans.
  • Assist with planning, scheduling, and revising the Minimum Data Set, including implementation of Resident Assessment Protocols and Triggers.
  • Report suspected or known fraud involving false billings, cost reports, or kickbacks.
  • Provide data to the Quality Assurance and Assessment Committee as requested.

PACS Group has appointed Carey P. Hendrickson as chief financial officer, effective 27 April 2026. Hendrickson brings nearly four decades of financial leadership across healthcare, senior living and media sectors. Most recently, Hendrickson served as CFO of U.S. Physical Therapy, overseeing 779 outpatient clinics across 44 states. PACS Group has announced that co-founder and chief financial officer Mark Hancock will retire on 30 June 2026. He will continue serving on the company's board of directors as vice chairman. Hancock co-founded PACS in 2013 with Jason Murray, starting with two post-acute care facilities in San Diego.
