Requirements
  • Requires an LPN/LVN, LSW, LCSW, or LMSW, or another non-RN license permitted by applicable state law.
  • Requires a Nursing Diploma or Associate of Science in Nursing or a related field, or an equivalent combination of education and experience.
  • Requires a minimum of 2 years of experience working with individuals with chronic illnesses, co-morbidities, and/or disabilities in a Service Coordinator or similar role.
  • Requires a current, unrestricted LPN/LVN, LSW, LCSW, LMSW, or other non-RN license permitted by applicable state law.
  • Decision-making requires understanding disease processes and terminology and applying clinical guidelines, without requiring nursing judgment.
Responsibilities
  • Assist the responsible registered nurse in identifying members at risk for high-risk complications.
  • Obtain clinical data as directed by the responsible registered nurse.
  • Assist the responsible registered nurse in identifying members who may benefit from an alternative level of care or other waiver programs.
  • Provide collected information to the responsible registered nurse for verification and interpretation, additional assessments, and development, monitoring, evaluation, and revision of member care plans.
  • Coordinate care for members with chronic illnesses, co-morbidities, and/or disabilities as directed by the responsible registered nurse and in conjunction with the registered nurse, member, and healthcare team.
  • Perform telephonic or face-to-face assessments within the scope of licensure to identify, evaluate, coordinate, and manage members' physical health, behavioral health, social-service, and long-term-services-and-supports needs.
  • Travel to client worksites and other locations as necessary.
Desired Qualifications
  • A Master of Arts or Master of Science in Health or Nursing is preferred.
  • State-specified certification may be required based on state law and/or contract.
